摘要
目的观察多囊卵巢综合征肝郁肾虚证采用炔雌醇环丙孕酮片并二甲地黄汤的临床效果。方法回顾性选取2022年8月—2024年1月长春中医药大学附属医院收治的120例多囊卵巢综合征肝郁肾虚证患者的临床资料。根据治疗方法不同分为对照组、观察组,各60例。对照组采用炔雌醇环丙孕酮片治疗,观察组采用炔雌醇环丙孕酮片并二甲地黄汤治疗。比较两组卵泡刺激素(fol⁃licle stimulating hormone,FSH)、抗苗勒氏管激素(anti-Müllerian hormone,AMH)、睾酮(testosterone,T)、促黄体生成素(luteinizing hormone,LH)、亲吻素(kisspeptin,Kp)水平、卵泡数目、成熟卵泡直径、子宫内膜厚度、子宫动脉血流阻力指数(resistance index,RI)、搏动指数(pulsitility index,PI)、空腹血糖(fasting plasma glucose,FPG)、空腹胰岛素(fasting insulin,FIN)、胰岛素抵抗指数(homeostasis model as⁃sessment insulin resistance,HOMA-IR)及不良反应发生情况。结果治疗后,两组FSH比较,差异无统计学意义(P>0.05)。治疗后,两组Kp均低于治疗前,差异有统计学意义(P均<0.05);但治疗后两组间Kp对比,差异无统计学意义(P>0.05)。治疗后,两组LH、T、AMH、卵泡数目、RI、PI、HOMA-IR、FPG、FIN水平均低于治疗前,成熟卵泡直径、子宫内膜厚度高于治疗前,且观察组均优于对照组,差异有统计学意义(P均<0.05)。观察组不良反应发生率为6.67%(4/60),与对照组的10.00%(6/60)比较,差异无统计学意义(χ^(2)=0.436,P>0.05)。结论多囊卵巢综合征肝郁肾虚证采用炔雌醇环丙孕酮片并二甲地黄汤能够调节性激素和生殖激素水平,恢复卵巢数目、形态及子宫内膜厚度,改善子宫动脉血流状态及内分泌功能,且不会增加治疗风险。
Objective To observe the clinical effect of using ethinylestradiol and cyproterone acetate tablets and Erjia Dihuang Decoction in the treatment of polycystic ovary syndrome with liver depression and kidney deficiency syndrome.Methods Clinical data of 120 patients with liver depression and kidney deficiency syndrome of polycystic ovary syndrome admitted to the Affiliated Hospital of Changchun University of Traditional Chinese Medicine from August 2022 to January 2024 were retrospectively selected.According to different treatment methods,they were divided into control group and observation group,60 cases each.The control group was treated with ethinylestradiol and cyproterone acetate tablets,and the observation group was treated with ethinylestradiol and cyproterone acetate tablets and Erjia Dihuang Decoction.The levels of follicle stimulating hormone(FSH),anti-Müllerian hormone(AMH),testosterone(T),luteinizing hormone(LH),kisspeptin(Kp),follicle number,mature follicle diameter,endometrial thickness,uterine artery blood flow resistance index(RI),pulsitility index(PI),fasting plasma glucose(FPG),fasting insulin(FIN),homeostasis model assessment insulin resistance(HOMA-IR)and adverse reactions were compared between the two groups.Results After tratment,there was no significant difference in FSH between the two groups(P>0.05).After treatment,Kp in the two groups werelower than before treatment,and the differences were statistically significant(both P<0.05).There was no significant difference in Kp between the two groups after treatment(P>0.05).After treatment,LH,T,AMH,follicle number,RI,PI,HOMA-IR,FPG and FIN levels in both groups were lower than before treatment,mature follicle diameter and endometrial thickness were higher than before treatment,and the above indexes in the observation group were better than those in the control group,the differences were statistically significant(all P<0.05).The incidence of adverse reactions was 6.67%(4/60)in the observation group,and 10.00%(6/60)in the control group,and the difference was statistically significant(χ^(2)=0.436,P>0.05).Conclusion The use of ethinylestradiol and cyproterone acetate tablets and Erjia Dihuang Decoction in the treatment of polycystic ovary syndrome with liver depression and kidney deficiency syndrome can regulate the levels of sex hormones and reproductive hormones,restore the number,shape,and thickness of ovaries,improve uterine artery blood flow status and endocrine function,and do not increase the risk of treatment.
