摘要
目的探究胰岛素、饮食疗法对妊娠期糖尿病患者血糖的影响情况。方法选择2022年1月—2024年3月太仓市第一人民医院收治的189例妊娠期糖尿病患者为研究对象,按照治疗方法不同,分为对照组(n=94,饮食疗法+地特胰岛素)、研究组(n=95,饮食疗法+门冬胰岛素)。对比两组血糖水平、胰岛功能、脂代谢指标、母婴不良结局。结果治疗后,研究组空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于对照组,差异有统计学意义(P均<0.05)。治疗后,研究组胰岛β细胞功能指数高于对照组,胰岛素抵抗指数低于对照组,差异有统计学意义(P均<0.05)。治疗后,研究组总胆固醇、甘油三酯、低密度脂蛋白胆固醇水平均低于对照组,高密度脂蛋白胆固醇水平高于对照组,差异有统计学意义(P均<0.05)。研究组母婴不良结局发生率低于对照组,差异有统计学意义(P<0.05)。结论胰岛素、饮食疗法在妊娠期糖尿病患者治疗中应用可降低血糖水平。
Objective To explore the effect of insulin and diet therapy on blood glucose in patients with gestational diabetes mellitus.Methods A total of 189 patients with gestational diabetes mellitus admitted to the First People's Hospital of Taicang from January 2022 to March 2024 were selected as the research objects and divided into control group(n=94,diet therapy+insulin detemir)and study group(n=95,diet therapy+insulin aspart)according to different treatment methods.The blood glucose levels,islet function,lipid metabolism indexes,maternal and infant adverse outcomes were compared between the two groups.Results After treatment,the levels of fasting blood glucose,2-hour postprandial blood glucose and glycated hemoglobin A1c in the study group were lower than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the isletβcell function index of the study group was higher than that of the control group,and the insulin resistance index was lower than that of the control group,the differences were statistically significant(both P<0.05).After treatment,the levels of total choles-terol,triglyceride and low density lipoprotein cholesterol in the study group were lower than those in the control group,and the levels of high density lipoprotein cholesterol was higher than that in the control group,the differences were sta-tistically significant(all P<0.05).The incidence of maternal and infant adverse outcomes in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The application of insulin and diet therapy in the treatment of patients with gestational diabetes can reduce blood glucose levels.
