摘要
目的:探讨改变轮模式下肺康复护理对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者肺康复训练依从性的影响。方法:选择2020年4月—2023年4月南通市通州区人民医院收治的80例COPD患者作为研究对象,采用摸球法分为对照组(n=40)和观察组(n=40)。对照组予以常规护理,观察组予以行为改变轮模式下肺康复护理,比较两组肺康复训练依从性、呼吸功能[圣乔治呼吸问卷(St.George's respiratory questionnaire,SGRQ)]、6分钟步行试验距离(six minutes walk test,6MWT)、生活质量[慢性阻塞性肺疾病评估量表(COPD assessment test,CAT)]、肺功能指标[呼气流量峰值(peak expiratory flow,PEF)、用力肺活量(forced vital capacity,FVC)]。结果:观察组肺康复训练总依从率为92.50%,高于对照组的75.00%,差异有统计学意义(P<0.05);观察组干预后活动受限、呼吸症状和疾病影响评分低于对照组,6MWT长于对照组,CAT评分低于对照组,PEF和FVC高于对照组,差异有统计学意义(P<0.05)。结论:行为改变轮模式下肺康复护理能显著提升COPD患者肺康复训练依从性、呼吸功能、运动耐力、生活质量及肺功能。
Objective:To investigate the effect of pulmonary rehabilitation nursing under behavioral change wheel model on the compliance of pulmonary rehabilitation training in patients with chronic obstructive pulmonary disease(COPD).Method:A total of 80 patients with COPD who admitted to Nantong Tongzhou People's Hospital from April 2020 to April 2023 were selected as the research objects,they were divided into the control group(n=40)and the observation group(n=40)by touching the ball.The control group was given conventional nursing care,while the observation group was given pulmonary rehabilitation nursing under behavior change wheel model.The compliance of pulmonary rehabilitation training,respiratory function[St.George's respiratory questionnaire(SGRQ)],6 minutes walk test(6MWT),quality of life[COPD assessment test(CAT)],lung function indexes[peak expiratory flow(PEF),forced vital capacity(FVC)]were compared between two groups.Result:The total compliance rate of pulmonary rehabilitation training in the observation group was 92.50%,which was higher than 75.00%in the control group,and the difference was statistically significant(P<0.05).The scores of activity limitation,respiratory symptoms and disease shadow in the observation group after intervention were lower than those in the control group,6MWT was longer than that in the control group,CAT score was lower than that in the control group,PEF and FVC were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Pulmonary rehabilitation nursing under behavior change wheel mode can significantly improve the compliance of pulmonary rehabilitation training,respiratory function,exercise endurance,quality of life and lung function in patients with COPD.
