摘要
目的总结智能闭环给药系统在海上执行援外人道主义医疗服务中的应用经验及效果。方法回顾性分析“和平方舟”医院船在执行“和谐使命-2023”任务中采用一款舰船专用智能闭环给药系统实施静脉全身麻醉的17例患者的病历资料,包括一般资料、手术类型、麻醉方式、麻醉时间、术中低血压发生率及麻醉相关并发症等,总结该智能给药系统在特殊场景麻醉中的应用优势。结果17例接受全身静脉麻醉下行手术或诊疗操作的患者,采用舰船专用智能闭环给药系统,通过麻醉深度实时监测反馈,多闭环调控静脉麻醉,实施静脉全身麻醉,其中男6例、女11例,年龄为(37.59±8.33)岁,BMI为(26.32±5.50)kg/m^(2),其中BMI值≥30 kg/m^(2)的有4例,最大值为36.71 kg/m^(2)。患者基础心率、气管插管前和后、手术开始后2 min的心率分别为(70.93±8.96)次/min、(62.53±6.53)次/min、(65.67±6.85)次/min和(61.00±6.69)次/min;基础血压、气管插管前和后、手术开始后2 min的平均动脉压分别为(85.67±9.14)mmHg(1 mmHg=0.133 kPa)、(77.47±7.94)mmHg、(79.93±8.44)mmHg和(78.33±8.73)mmHg,所有患者麻醉诱导平稳,气管插管顺利,麻醉维持期间麻醉深度满意,术后苏醒顺利,未发生苏醒延迟、术后恶心呕吐和谵妄等麻醉相关不良事件。结论医院船在执行海外医疗服务中,智能闭环给药系统能协助麻醉医师实施精准麻醉,维持患者满意的麻醉深度及稳定循环,通过减轻工作负荷缓解特殊场景下麻醉医师不足的临床困境。
Objective To summarize the effectiveness of an intelligent closed-loop drug delivery system in overseas humanitarian medical services.Methods The medical records of 17 patients who underwent intravenous general anesthesia using a intelligent closed-loop drug delivery system on the Peace Ark hospital ship during the Harmonious Mission-2023 were retrospectively analyzed.The data included general information,surgical type,anesthesia methods,anesthesia time,incidence of intraoperative hypotension,and anesthesia-related complications.The advantages of the intelligent drug delivery system were summarized.Results There were 6 males and 11 females,with an average age of(37.59±8.33)years and a BMI of(26.32±5.50)kg/m^(2).BMI was over 30 kg/m^(2) in 4 patients,with a maximum value of 36.71 kg/m^(2).The heart rate was(70.93±8.96)beat/min,(62.53±6.53)beat/min,(65.67±6.85)beat/min,and(61.00±6.69)beat/min at basic level,before intubation,after intubation,and 2 minutes after the start of surgery,respectively.The arterial pressure was(85.67±9.14)mmHg,(77.47±7.94)mmHg,(79.93±8.44)mmHg,and(78.33±8.73)mmHg at these time points,respectively.All the patients had successful anesthesia induction and tracheal intubation,satisfactory anesthesia depth during maintenance,and quick recovery after surgery.No anesthesia-related complications,such as delayed recovery,postoperative nausea and vomiting,or delirium,occurred.Conclusion In the implementation of overseas medical services on the hospital ships,the intelligent closed-loop drug delivery system can assist anesthesiologists in performing precise anesthesia and maintaining satisfactory anesthesia depth,and can help handle clinical difficulties such as insufficient anesthesiologists in special scenarios by reducing workloads.
