摘要
目的评价岭南飞针疗法对脑卒中后肢体痉挛性偏瘫患者的肢体功能和生活能力的影响。方法将符合纳入标准的60例患者按随机数字表法分为治疗组和对照组,每组30例。两组均给予中风后常规药物结合康复训练的基础治疗,同时治疗组采用岭南飞针疗法治疗原则选取穴位并进行相关治疗,对照组采用传统针刺法结合头针法治疗,每周治疗6次,每天1次,共3周。分别在治疗前后比较两组改良Ashworth量表(modified Ashworth scale,MAS)、简化Fugl-Meyer量表(Fugl-Meyer assessment scale,FMA)及改良Barthel指数(Barthel index,MBI)评分差异,并比较两组的临床疗效及愈显率。结果治疗前两组MAS、FMA、MBI评分比较差异均无统计学意义(P>0.05),具有可比性。治疗后,两组患者MAS痉挛评分均明显降低(P<0.01),且治疗组优于对照组(P<0.05);两组患者FMA评分及MBI评分均有明显提高(P<0.01),且治疗组高于对照组(P<0.05);治疗组的总有效率为93.33%(28/30),而对照组为73.33%(22/30),差异有统计学意义(P<0.05);治疗组的愈显率为60.00%(18/30),对照组的愈显率为33.33%(10/30),差异有统计学意义(P<0.05)。结论岭南飞针疗法能改善卒中后痉挛性瘫痪患者肌张力,有效减轻肢体痉挛程度,促进肢体运动功能恢复,提高运动能力及日常生活能力,且临床效果肯定,在治疗后短期内即可取得较显著效果。
Objective The objective of this study was to examine the effect of Lingnan flying needle therapy on spastic hemiplegia after stroke.Methods The patients of spastic hemiplegia post-stroke were divides into two groups equally.The control group received conventional medication,rehabilitation and acupuncture treatment,and the observation group received conventional medication and rehabilitation in combined with Lingnan flying needle therapy.The scores of modified Ashworth scale(MAS),Fugl-Meyer assessment scale(FMA)and improved Barthel index(MBI)and total effect rate after 3 weeks of treatment were compared.Results The scores of MAS,FMA and MBI were not significantly different before treatment(P>0.05).After treatment,the MAS scores were significantly lower in both groups(P<0.01),and the MAS score of the treatment group was better than that of the control group(P<0.05).The FMA score and MBI score were improved significantly in both groups(P<0.01),and the FMA score and MBI score of the treatment group were higher than those of the control group(P<0.05).The overall response rate in the treatment group was 93.33%(28/30),and that was 73.33%(22/30)in the control group.The difference was statistically significant(P<0.05).The increase of predominance rate in the treatment group was 60.00%(18/30)and that of the control group was 33.33%(10/30).The difference was statistically significant(P<0.05).Conclusion Lingnan flying needle therapy is effective for spastic hemiplegia post-stroke.It can effectively reduce the degree of limb spasm and improve the exercise ability and the activity of daily living.
