血清微小RNA-582-5p和微小RNA-215-5p在肺结核患者中的表达及临床意义

The expression and clinical significance of serum miR-582-5p and miR-215-5p in patients with pulmonary tuberculosis reference significance for clinical diagnosis of pulmonary tuberculosis.

摘要 目的探讨血清微小RNA(microRNA,miR)-582-5p和miR-215-5p在肺结核患者中的表达水平及其临床意义。方法选取2022年3月至2024年5月重庆医科大学附属第二医院石柱分院收治确诊的肺结核患者127例为结核组,另选取同时期在我院体检的96例健康体检志愿者为对照组。2组受试者在年龄、性别方面差异均无统计学意义(P均>0.05)。采用定量聚合酶链式反应(reverse transcription-polymerase chain reaction,RT-PCR)检测受试者血清miR-582-5p和miR-215-5p表达水平,采用全自动生化分析仪检测血清红细胞沉降速率(erythrocyte sedimentation rate,ESR)、C反应蛋白(C-reactiveprotein,CRP)、干扰素γ(interferon-γ,IFN-γ)、白细胞介素(interleukin,IL)-12水平;Pearson法分析肺结核患者血清miR-582-5p和miR-215-5p相关性及2者与CRP、ESR、INF-γ、IL-12的相关性;采用受试者工作特征(receiver operating characteristic,ROC)曲线分析血清miR-582-5p和miR-215-5p表达水平诊断肺结核的效能。结果肺结核组的CRP、ESR、INF-γ、IL-12水平显著高于对照组(P均<0.05)。肺结核组血清miR-582-5p表达水平显著高于对照组,而血清miR-215-5p表达水平显著低于对照组(P均<0.05)。Pearson法分析显示,肺结核患者血清miR-582-5p和miR-215-5p表达水平呈负相关(r=-0.490,P=0.000),血清miR-582-5p表达水平与CRP(r=0.415,P=0.000)、ESR(r=0.485,P=0.000)、IFN-γ(r=0.514,P=0.000)、IL-12(r=0.358,P=0.019)水平呈正相关,血清miR-215-5p表达水平与CRP(r=-0.341,P=0.021)、ESR(r=-0.391,P=0.017)、INF-γ(r=-0.583,P=0.000)、IL-12(r=-0.416,P=0.000)水平呈负相关。ROC曲线显示血清miR-582-5p和miR-215-5p、2者联合诊断肺结核的ROC曲线下面积分别为0.810、0.788、0.865。2者联合诊断优于血清miR-582-5p和miR-215-5p各自单独诊断(z_(miR-582-5p-2者联合)=2.232、z_(miR-215-5p-2者联合)=2.263,P=0.026、0.024)。结论检测机体内血清miR-582-5p和miR-215-5p表达水平将为筛查肺结核患者提供新的生物学标志物,对临床诊断肺结核可以提供一定的参考意义。 Objective To explore the expression and clinical significance of serum miR-582-5p and miR-215-5p in patients with pulmonary tuberculosis infection.Methods From March 2022 to May 2024,127 confirmed pulmonary tuberculosis patients admitted to our hospital were collected,and 96 healthy volunteers who underwent physical examinations at our hospital were selected.There was no obvious difference in age and gender between the two groups.Quantitative polymerase chain reaction(Reverse Transcription-Polymerase Chain Reaction,Reverse Transcription-Polymerase Chain Reaction,RT-PCR)was applied to detect the expression levels of serum miR-582-5p and miR-215-5p in patients.Fully automated biochemical analyzer was applied to detect serum erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),IFN-γ,and IL-12 levels.Pearson method was applied to analyze the correlation between serum miR-582-5p and miR-215-5p in patients with pulmonary tuberculosis,and their correlation with CRP,ESR,IFN-γ,and IL-12.The predictive efficacy of serum miR-582-5p and miR-215-5p expression levels in the diagnosis of pulmonary tuberculosis was analyzed using ROC curves.Results The levels of CRP,ESR,IFN-γ,and IL-12 in the tuberculosis group were obviously higher than those in the control group(P<0.05).The expression level of serum miR-582-5p in the tuberculosis group was obviously higher than that in the control group(P<0.05),while the serum miR-215-5p in the tuberculosis group was obviously lower than that in the control group(P<0.05).Pearson analysis showed that the expression levels of serum miR-582-5p and miR-215-5p in patients with pulmonary tuberculosis were negatively correlated(r=-0.490,P=0.000),the expression level of serum miR-582-5p was positively correlated with CRP(r=0.415,P=0.000)、ESR(r=0.485,P=0.000)、IFN-γ(r=0.514,P=0.000)、IL-12(r=0.358,P=0.019)levels,while the expression level of serum miR-215-5p was negatively correlated with CRP(r=-0.341,P=0.021)、ESR(r=-0.391,P=0.017)、IFN-γ(r=-0.583,P=0.000)、IL-12(r=-0.416,P=0.000).levels(P<0.05).ROC curve showed that the area under the curve(AUC)of serum miR-582-5p and miR-215-5p,and their combination in the diagnosis of pulmonary tuberculosis was 0.810,0.788,and 0.865,respectively.The combination of the 2 was superior to the individual diagnosis of serum miR-582-5p and miR-215-5p(z_(miR582-5p-combination)=2.232,z_(miR-215-5p-combination)=2.263,P=0.026,0.024).Conclusion Detecting the expression levels of serum miR-582-5p and miR-215-5p in the body will provide new biological markers for screening patients with pulmonary tuberculosis,and can provide certain reference significance for clinical diagnosis of pulmonary tuberculosis.
