摘要
目的:分析心脏瓣膜置换术后患者预后不良的影响因素。方法:选取2021年1月至2023年1月该院收治的198例重症心脏瓣膜病患者进行横断面研究,统计心脏瓣膜置换术后患者预后情况,采用Logistic回归分析心脏瓣膜置换术后患者预后不良的影响因素。结果:198例患者心脏瓣膜置换术后预后不良24例,发生率为12.12%,设为预后不良组,其余设为预后良好组。预后不良组年龄≥60岁、美国纽约心脏病协会(NYHA)心功能分级为Ⅲ~Ⅳ级、主动脉阻断时间≥120 min、体外循环时间≥150 min、术后发生感染等占比均高于预后良好组,差异有统计学意义(P<0.05);多因素Logistic回归分析结果显示,年龄≥60岁、NYHA心功能分级为Ⅲ~Ⅳ级、主动脉阻断时间≥120 min、体外循环时间≥150 min、术后发生感染等均为心脏瓣膜置换术后患者预后不良的危险因素(OR>1,P<0.05)。结论:年龄≥60岁、NYHA心功能分级为Ⅲ~Ⅳ级、主动脉阻断时间≥120 min、体外循环时间≥150 min、术后发生感染等均为心脏瓣膜置换术后患者预后不良的危险因素。
Objective:To analyze influencing factors of poor prognosis in patients after cardiac valve replacement.Methods:A cross-sectional study was conducted on 198 patients with severe valvular heart disease admitted to the hospital from January 2021 to January 2023.The prognosis of these patients after cardiac valve replacement was statistically analyzed.Logistic regression was used to analyze the influencing factors of poor prognosis in the patients after cardiac valve replacement.Results:There were 24 patients with poor prognosis after cardiac valve replacement in the 198 patients,the incidence rate was 12.12%,they was set as the poor prognosis group,and the rest were set as the good prognosis group.The proportions of the patients with age≥60 years old,the New York Heart Association(NYHA)cardiac function grade III-IV,aortic occlusion time≥120 min,extracorporeal circulation time≥150 min,and postoperative infection in the poor prognosis group were higher than those in the good prognosis group,and the differences were statistically significant(P<0.05).Multivariate Logistic analysis showed that age≥60 years,NYHA cardiac function grade III-IV,aortic occlusion time≥120 min,extracorporeal circulation time≥150 min,and postoperative infection were all risk factors for poor prognosis in the patients after cardiac valve replacement(OR>1,P<0.05).Conclusions:Age≥60 years old,NYHA cardiac function grade III-IV,aortic occlusion time≥120 min,extracorporeal circulation time≥150 min,and postoperative infection are the risk factors for poor prognosis in the patients after cardiac valve replacement.
