摘要
目的探讨水负荷参数与维持性血液透析(MHD)患者自体动静脉内瘘(AVF)狭窄的相关性。方法51例MHD患者根据AVF超声结果为AVF狭窄组(n=26)与AVF正常组(n=25)。记录患者的年龄、性别、透析龄、收缩压、舒张压、Kt/V;费森人体成分分析仪测定水负荷参数(OH值)。通过Spearman相关分析MHD患者指标与AVF狭窄的相关性,多因素Logistic回归分析MHD患者AVF狭窄的影响因素,利用受试者工作特征(ROC)曲线分析OH值对AVF狭窄的预测效能。结果AVF狭窄组年龄、透析龄、SBP及OH高于AVF无狭窄组,Kt/V低于AVF无狭窄组,差异有统计学意义(均P<0.05)。Spearman相关分析结果显示Kt/V与AVF内径呈正相关;透析龄、SBP、OH与AVF内径呈负相关。Logistic回归结果表明OH及SBP与AVF狭窄发生显著正相关,Kt/V与AVF狭窄显著负相关。ROC曲线证明OH值对AVF狭窄有预测价值。结论MHD患者AVF狭窄与体内容量负荷过重存在相关性,OH值对MHD患者内瘘狭窄有一定预测价值。
Objective To explore the correlation between overhydrate and stenosis of autologous arteriovenous fistula(AVF)in maintenance hemodialysis(MHD)patients.Methods A total of 51 MHD patients who underwent AVF ultrasound examination were divided into AVF stenosis group(n=26)and AVF normal group(n=25)according to the ultrasound results.Age,gender,dialysis vintage,systolic blood pressure,diastolic blood pressure and Kt/V were recorded.The overhydrate(OH)values were measured by the Fresenius body composition analyzer.Spearman correlation analysis was used to analyze the correlation between MHD patient indicators and AVF stenosis.Multivariate logistic regression analysis was used to analyze the influencing factors of AVF stenosis in MHD patients.The receiver operating characteristic(ROC)curve was used to analyze the predictive efficacy of OH value for AVF stenosis.Results Age,dialysis vintage,SBP,and OH were higher in the AVF stenosis group than those in the AVF normal group,while Kt/V was lower in the AVF stenosis group(all P<0.05).Spearman correlation analysis showed that Kt/V was positively correlated with AVF inner diameter,while dialysis periods,SBP,and OH were negatively correlated with AVF inner diameter.Logistic regression results showed that OH and SBP were significantly positively correlated with the occurrence of AVF stenosis,while Kt/V was significantly negatively correlated with AVF stenosis.The ROC curve demonstrated that the OH value had predictive value for AVF stenosis.Conclusion There is a correlation between AVF stenosis in MHD patients and excessive body fluid load.The OH value has a certain predictive value for AVF stenosis in MHD patients.
