摘要
糖尿病肾病是糖尿病最常见的微血管并发症之一,起因尚不明确。随着糖尿病发病率的升高,糖尿病肾病患病率亦逐渐增加,病至后期,常致肾功能不全或肾衰竭,严重危害患者身心健康,因此对于糖尿病早期的干预十分重要。张定华主任医师认为消渴肾病乃消渴日久、气阴两虚基础上发展而来,认为气阴两虚、络脉瘀滞,阴阳两虚、玄府失司是消渴肾病的基本病机,治疗糖尿病肾病前期多以滋阴补肾为主,辅以祛瘀通络、利水开玄等法。通过阐述糖尿病肾病的中医病因病机,总结张定华主任医师治疗糖尿病肾脏病的学术思想和临床经验,并附验案一则。
Diabetic kidney disease(DKD)is one of the common microvascular complications of diabetes,the cause of DKD is still unclear.With the increaseing of the incidence of DM,the prevalence of DKD has gradually increased.In the end-stage of DKD,it often causes renal insufficiency or renal failure,which seriously endangers the physical and mental health of patients.Therefore,early intervention in diabetes is more important.Chief Physician Zhang Dinghua believes that DKD is developed on the basis of diabetes for a long time,the basic pathogenesis of DKD are the deficiency of both Qi and Yin plus blood stasis,deficiency of both Yin and Yang and Xuanfu′s dysfunction in opening and closing.In the treatment of early-stage DKD,it is mainly based on nourishing Kidney-Yin,supplemented by methods such as activating blood and eliminating disease,promoting urination andopening the Xuan-Fu.This article briefly explained the etiology and pathogenesis of DKD in traditional Chinese medicine,shares Chief Physician Zhang Dinghua's academic theory and clinical experience in the treatment of DKD,and attaches a medical case.
