地舒单抗联合硫辛酸对骨质疏松症患者HMGB-1、Lp-PLA2、N-MID、Omentin-1水平的影响

Effects of disumab combined with lipoic acid on levels of HMGB-1,Lp-PLA2,N-MID and Omentin-1 in patients with osteoporosis

摘要 目的 观察地舒单抗联合硫辛酸对骨质疏松症患者高迁移率族蛋白-1(high mobility group protein-1,HMGB-1)、脂蛋白相关磷脂酶A2(lipoprotein-associated phospholipase A2,Lp-PLA2)、骨钙素N段中分子片段(N-MID)、网膜素1(Omentin-1)水平的影响。方法 选择2020年10月至2023年10月于洛阳市东方人民医院治疗的62例骨质疏松症患者,符合标准患者入组研究,以随机数字表法分组,地舒单抗组31例,进行地舒单抗注射液治疗,硫辛酸联合组31例,地舒单抗注射液治疗同时服用硫辛酸胶囊治疗,治疗前、后检测患者血清Omentin-1、肿瘤坏死因子-α(TNF-α)、单胺氧化酶A(MAOA)、Lp-PLA2、高级氧化蛋白产物(AOPP)、HMGB-1、核因子кB受体活化因子配基(RANKL)、活性氧(ROS)、N-MID水平,检测骨密度值(腰椎L1-L4),给予6点行为评分法(BRS-6)评价,疗程结束后比较硫辛酸联合组、地舒单抗组临床疗效。结果 硫辛酸联合组Lp-PLA2、TNF-α、HMGB-1水平较地舒单抗组低(P<0.05);硫辛酸联合组AOPP、MAOA、ROS水平较地舒单抗组低(P<0.05);硫辛酸联合组RANKL、N-MID水平低于地舒单抗组(P<0.05),硫辛酸联合组Omentin水平高于地舒单抗组(P<0.05);硫辛酸联合组BRS-6评分低于地舒单抗组(P<0.005),硫辛酸联合组骨密度值高于地舒单抗组(P<0.005);硫辛酸联合组总有效率为96.77%,地舒单抗组总有效率为74.19%,硫辛酸联合组患者总有效率较地舒单抗组高(P<0.05)。结论 地舒单抗联合硫辛酸对骨质疏松症患者,可减少氧化应激及炎症损伤,调节骨代谢,提升骨密度及临床疗效,缓解疼痛。 Objective To observe the effects of desumab combined with lipoic acid on the levels of high mobility group protein-1(HMGB-1),lipoprotein-associated phospholipase A2(Lp-PLA2),N-MID of osteocalcin and Omentin-1 in patients with osteoporosis.Methods Sixty-two patients with osteoporosis who were treated in our hospital were selected in the study from October 2020 to October 2023.The patients who met the criteria were divided into two groups by random number table method.31 cases in the desumab group received desumab injection,31 cases in the lipoic acid combination group were given desumab injection and lipoic acid capsule treatment.Serum levels of Omentin-1,tumor necrosis factor-α(TNF-α),monoamine oxidase A ( MAOA), Lp - PLA2, advanced oxidized protein product (AOPP), HMGB - 1, nuclear factor cask B receptor activating factor ligand (RANKL), reactive oxygen species ( ROS) and N - MID were detected before andafter treatment. Bone mineral density (lumbar L1 - L4) was detected and evaluated by 6 - point behavior score(BRS - 6). The clinical efficacy of lipoic acid combined group and desumab group was compared after treatment. Results The levels of Lp - PLA2, TNF - α and HMGB - 1 in lipoic acid combination group were lowerthan those in disumab group (P < 0. 5). The levels of AOPP,MAOA and ROS in lipoic acid combination groupwere lower than those in desumab group (P < 0. 5). The levels of RANKL and N - MID in lipoic acid combinedgroup were lower than those in disumab group ( P < 0. 5), and the levels of Omentin in lipoic acid combinedgroup were higher than those in disumab group ( P < 0. 5). The BRS - 6 score of lipoic acid combined groupwas lower than that of disumab group ( P < 0. 005), and the BMD of lipoic acid combined group was higherthan that of disumab group (P < 0. 005). The total effective rate of lipoic acid combined group was 96. 77% ,and that of desumab group was 74. 19% . The total effective rate of lipoic acid combined group was higher thanthat of desumab group (P < 0. 5). Conclusion Disumab combined with lipoic acid can reduce oxidative stressand inflammatory damage, regulate bone metabolism, improve bone mineral density and relieve pain in patientswith osteoporosis.
