摘要
目的探讨早期亚低温联合咪达唑仑应用于难治性癫痫持续状态患者中的效果。方法选取2022年1月至2023年12月山东第二医科大学附属诸城市人民医院收治的52例难治性癫痫持续状态患者作为研究对象,按照随机数字表法分为对照组与观察组,每组26例。对照组采用咪达唑仑治疗,观察组在此基础上加用早期亚低温治疗。比较两组患者的氧化应激指标、脑神经相关指标、癫痫持续状态严重程度评分量表(STESS)、格拉斯哥转归量表评分(GOS)、临床疗效、不良反应发生情况。结果治疗后,观察组的一氧化氮(NO)、丙二醛(MDA)水平以及甘丙肽(GAL)、神经肽Y(NPY)、脑源性神经营养因子(BDNF)水平均低于对照组,超氧化物歧化酶(SOD)水平与总有效率高于对照组,差异有统计学意义(P<0.05)。观察组治疗后的STESS评分低于对照组,GOS评分高于对照组,差异有统计学意义(P<0.05)。两组患者的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论对难治性癫痫持续状态患者实施早期亚低温联合咪达唑仑治疗具有较好的效果,有助于缓解氧化应激状态以及保护脑功能,且具有一定的安全性。
Objective To investigate the effect of early mild hypothermia combined with Midazolam in patients with refractory status epilepticus.Methods From January 2022 to December 2023,52 patients with refractory status epilepticus admitted to Zhucheng People's Hospital Affiliated to Shandong Second Medical University were selected as research objects,and they were divided into control group and observation group according to random number table method,with 26 cases in each group.The control group was treated with Midazolam,and the observation group was treated with early mild hypothermia.The oxidative stress indexes,cranial-nerve related indexes,status epilepticus severity score(STESS),Glasgow outcome scale(GOS),clinical efficacy and occurrence of adverse reactions were compared between the two groups.Results After treatment,the levels of nitric oxide(NO),malondialdehyde(MDA),galanin(GAL),neuropeptide Y(NPY)and brain-derived neurotrophic factor(BDNF)in observation group were lower than those in control group,and the level of superoxide dismutase(SOD)and total effective rate were higher than those in control group,with statistical significances(P<0.05).STESS score of observation group after treatment was lower than that of control group,GOS score was higher than that of control group,the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Early mild hypothermia combined with Midazolam has a good effect on refractory status epilepticus patients,which is helpful to alleviate oxidative stress and protect brain function,and has a certain safety.
