睡眠呼吸暂停合并心外膜脂肪异常在射血分数保留的心力衰竭研究中的进展

Research progress on sleep apnea combined with epicardial fat abnormalities in heart failure with preserved ejection fraction

摘要 射血分数保留的心力衰竭(HFpEF)特征是继发于舒张功能障碍的左心室充盈压升高。阻塞性睡眠呼吸暂停(OSA)通过多种复杂的机制引起心肌重构,与HFpEF的发展、预后密切相关。同时,心外膜脂肪组织(EAT)因其特定的解剖位置以及它分泌的多种炎症因子,都有可能促进HFpEF的发生。基于此,本文将近年来有关OSA、EAT及两者异常时对HFpEF的影响及相关机制、相关治疗进展进行综述。 Heart failure with preserved ejection fraction(HFpHF)is characterized by increased filling pressure in the left ventricle secondary to diastolic dysfunction.Obstructive sleep apnea(OSA)causes myocardial remodeling through complex mechanisms,which is closely related to the development and prognosis of HFpHF.Meanwhile,epicardial adipose tissue(EAT)may promote the occurrence of HFpEF due to its secretion of various inflammatory factors.Based on these,this article will review the impact and related mechanisms of OSA and EAT on HFpEF in recent years,as well as the progress in related treatment.
