摘要
基于李东垣“阴火”理论探讨多囊卵巢综合征高雄激素血症的病机与辨治。高雄激素血症是多囊卵巢综合征发生发展过程中的核心生化及临床表征之一,积极干预患者雄激素过多状态对纠正生殖内分泌紊乱及防治远期并发症等方面具有重要意义。中医学认为,脏腑功能失调,气滞火热,痰瘀交阻为多囊卵巢综合征的主要病机。李东垣的“阴火”理论可以是多囊卵巢综合征高雄激素血症病机的高度概括,脾胃虚弱是疾病发生之根本,阴火炽盛是疾病的病理表征,痰浊瘀血是疾病加重及缠绵的关键因素,治疗上以甘温补中、升阳泻火为主,兼以化痰逐瘀、补肾调冲,以达标本兼顾、治病求本之功。
Based on Li Dong Yuan s theory of“Yin fire”,we discuss the pathogenesis and treatment ofhyperandrogenemia of polycystic ovary syndrome.Hyperandrogenemia is one of the core biochemical and clinical signs in the development of polycystic ovary syndrome,and active intervention in patients with hyperandrogenism is of great significance in correcting reproductive endocrine disorders and preventing long-term complications.According to traditional medicine,dysfunction of internal organs,stagnation of qi and fire-heat,phlegm and stasis are the main pathologies of polycystic ovary syndrome.Li Dongyuan s“yin and fire”theory is a high degree of polycystic ovary syndrome hyperandrogenemia pathogenesis.We argue that the disease is rooted in a deficiency of spleen and stomach,that Yin fire inflammation is a pathological manifestation of the disease,and that blood stagnation and phlegm are the primary causes of the diseases aggravation and prolonged course.In order to cure the disease byaddressing both symptoms and root causes,the main treatment method is to tonify the spleen and stomach withsweet and warm-natured drugs,raise the Yang qi and diarrhea the Yin fire,and remove phlegm and eliminate stasis,nourish kidney and regulate thoroughfare.
