摘要
目的:分析卵巢畸胎瘤由经阴道彩色多普勒超声(TVS)联合经腹彩色多普勒超声(TAS)诊断的一致性和准确度。方法:回顾性分析2021年1月-2023年10月西华县人民医院85例卵巢畸胎瘤患者的临床资料。所有患者入院后均行TVS、TVS检查。以手术病理检查结果作为金标准,分析比较TVS、TVS联合TAS诊断卵巢畸胎瘤的一致性及诊断价值。结果:手术病理检查结果显示,85例卵巢畸胎瘤患者中78例为成熟卵巢畸胎瘤,7例为未成熟卵巢畸胎瘤。TVS诊断卵巢畸胎瘤的一致性较强(Kappa=0.635,P<0.05),TVS联合TVS诊断卵巢畸胎瘤的一致性强(Kappa=0.927,P<0.05),TVS联合TVS诊断卵巢畸胎瘤的灵敏度、特异度、准确度、阳性预测值、阴性预测值均略高于TVS,但差异无统计学意义(P>0.05)。结论:TVS联合TAS诊断卵巢畸胎瘤结果与手术结果的一致性强。
Objective:To analyze the consistency and accuracy of transvaginal color Doppler ultrasound(TVS)combined with transabdominal color Doppler ultrasound(TAS)in the diagnosis of ovarian teratoma.Methods:The clinical data of 85 patients with ovarian teratoma in Xihua County People's Hospital from January 2021 to October 2023 were retrospectively analyzed.All patients were examined by TVS and TVS after admission.The consistency and diagnostic value of TVS and TVS combined with TAS in the diagnosis of ovarian teratoma were analyzed and compared with the results of surgical pathology as the gold standard.Results:Pathological examination showed that 78 of the 85 patients with ovarian teratoma were mature and 7 were immature.The consistency of TVS in the diagnosis of ovarian teratoma was strong(Kappa value=0.635,P<0.05),and the consistency of TVS combined with TVS in the diagnosis of ovarian teratoma was strong(Kappa value=0.927,P<0.05).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of TVS combined with TVS in the diagnosis of ovarian teratoma were slightly higher than those of TVS,but the difference was not statistically significant(P>0.05).Conclusion:The results of TVS combined with TAS in the diagnosis of ovarian teratoma are consistent with the results of operation.
