摘要
目的探讨集束化护理干预措施对鼻咽癌放疗后口干症的影响。方法将2022年1月至2023年12月在江西省肿瘤医院头颈放疗科住院接受放射治疗200例患者采用配对设计队列原则分为观察组和对照组,每组100例。观察组采用集束化护理方案,对照组采用日常护理措施。评估和分析集束化护理干预在NPC放疗中对减少放射性口干症的有效性和安全性。结果所有患者临床病历资料完整且均纳入统计研究。观察组患者放射性口干症发生率为41%(41/100),对照组为55%(55/100),差异有统计学意义(χ^(2)=9.233,P=0.026)。亚组分析中,肿瘤N分期和患者是否接受同期放化疗是影响两组差异的主要因素。集束化护理干预措施能明显减少高N分期和采用同期放化疗患者的口干症发生率(χ^(2)=12.469,P=0.005;χ^(2)=5.322,P=0.028)。高级别口干症发生率在N分期亚组中差异有统计学意义(χ^(2)=6.358,P=0.014)。集束化护理干预措施同样能减少放疗后口腔黏膜炎的发生(χ^(2)=7.918,P=0.047)。结论集束化护理干预措施能减少高N分期和同期放化疗患者的口干症发生率,且减少口腔黏膜炎发生和提高了生活质量。
Objective To investigate the impact of bundled nursing interventions on the incidence of xerostomia in nasopharyngeal carcinoma(NPC)patients following radiotherapy.Methods A cohort study with a paired design was conducted on 200 NPC patients who received radiotherapy at the Department of Head and Neck Oncology,Jiangxi Cancer Hospital,from January 2022 to December 2023.Patients were divided into an intervention group(n=100)receiving bundled nursing care and a control group(n=100)receiving routine nursing care.The effectiveness and safety of the bundled nursing interventions in reducing radiotherapy-induced xerostomia were evaluated and analyzed.Results Clinical records for all patients were complete and included in the statistical analysis.The incidence of radiotherapy-induced xerostomia was 41%in the intervention group compared to 55%in the control group,a statistically significant difference(χ^(2)=9.233,P=0.026).Subgroup analysis indicated that tumor N-stage and concurrent chemoradiotherapy were primary influencing factors on xerostomia incidence between groups.Bundled nursing interventions significantly reduced xerostomia incidence in patients with high N-stage disease and those undergoing concurrent chemoradiotherapy(χ^(2)=12.469,P=0.005;χ^(2)=5.322,P=0.028).A significant reduction in high-grade xerostomia was also observed within the N-stage subgroup(χ^(2)=6.358,P=0.014).Moreover,the bundled nursing interventions effectively decreased the occurrence of oral mucositis post-radiotherapy(χ^(2)=7.918,P=0.047).Conclusion Bundled nursing interventions can significantly reduce the incidence of xerostomia in high N-stage NPC patients and those undergoing concurrent chemoradiotherapy.Additionally,these interventions are beneficial in lowering the occurrence of oral mucositis,contributing to improved quality of life.
