摘要
目的评估针药结合调控原发性高血压患者短中长期血压变异性的有效性,观察其降压效果及临床伴随症状的改善效果。方法选取2022年5月—2022年11月就诊于江苏省中医院针灸康复科及心内科门诊的64例原发性高血压患者,随机分为治疗组和对照组各32例。对照组予苯磺酸氨氯地平片口服结合一般治疗(高血压健康宣教),治疗组在对照组基础上加予电针治疗,两组均治疗8 w。观察两组治疗前后24 h动态血压、家庭自测血压、诊室血压及高血压生活质量量表对比等。结果治疗8周后治疗组患者ABPM中24hBPV、dBPV、nBPV均较对照组降低,差异具有统计学意义(P<0.05);HBPM中M-SSD、M-DSD、M-SCV、M-DCV均较对照组降低,差异具有统计学意义(P<0.05);诊室血压中L-SSD、L-DSD、L-SCV、L-DCV均较对照组降低,差异具有统计学意义(P<0.05);治疗组治疗结束后杜氏高血压生活质量量表评分(生理症状、睡眠状况、生气与活力、焦虑、压抑、工作状态、敌对分数维度)均较治疗前升高,差异具有统计学意义(P<0.05);均较对照组升高,差异具有统计学意义(P<0.05)。结论针药结合可以有效改善原发性高血压患者的短中长期血压变异性,提升患者降压平稳性,有效改善临床伴随症状,提高生活质量。
Objective To evaluate the effectiveness of acupuncture combined with medication in regulating short,medium,and long-term blood pressure variability in patients with primary hypertension,and to observe its antihypertensive effect and improvement of clinical accompanying symptoms.Methods From May 2022 to November 2022,64 patients with primary hypertension who were treated in the acupuncture and moxibustion Rehabilitation Department and the Cardiology Department of Jiangsu Provincial Hospital of Chinese Medicine were selected and randomly divided into the treatment group and the control group,with 32 patients in each group.The control group received oral amlodipine besylate tablets combined with general treatment(hypertension health education),while the treatment group received electroacupuncture treatment in addition to the control group.Both groups were treated for 8 weeks.Observe the comparison of 24-hour dynamic blood pressure,home self measured blood pressure,clinic blood pressure,and hypertension quality of life scale between two groups before and after treatment.Results After 8 weeks of treatment,the 24-hour BPV,dBPV,and nBPV in the ABPM of patients in the treatment group were significantly lower than those in the control group(P<0.05);The levels of M-SSD,M-DSD,M-SCV,and M-DCV in HBPM were significantly lower than those in the control group(P<0.05);The blood pressure levels of L-SSD,L-DSD,L-SCV,and L-DCV in the consultation room were significantly lower than those in the control group(P<0.05);The scores of the DU's Hypertension Quality of Life Scale(physiological symptoms,sleep status,anger and vitality,anxiety,depression,work status,hostility score dimensions)in the treatment group increased significantly after treatment compared to before treatment,and the difference was statistically significant(P<0.05);Both groups showed an increase compared to the control group,and the difference was statistically significant(P<0.05).Conclusion The combination of acupuncture and medication can effectively improve the short,medium,and long-term blood pressure variability in patients with primary hypertension,enhance the stability of blood pressure reduction,effectively improve clinical accompanying symptoms,and improve quality of life.
