摘要
目的:探究HA380血液灌流联合连续性肾脏替代治疗脓毒症合并急性肾损伤(AKI)的效果及对血流动力学的影响。方法:选取2023年1月—2024年2月贵州医科大学第二附属医院收治的脓毒症合并AKI患者102例为研究对象,采用随机数字表法分为对照组与观察组,每组51例。对照组实施连续性肾脏替代治疗,观察组在对照组基础上联合HA380血液灌流治疗。比较两组治疗效果、血流动力学指标水平、炎性指标水平及T淋巴细胞亚群情况。结果:观察组治疗总有效率高于对照组(P=0.041);治疗后,两组平均动脉压、中心静脉压水平升高,心率水平降低,观察组平均动脉压、中心静脉压水平高于对照组,心率水平低于对照组(P<0.05);治疗后,两组白细胞介素-6、C反应蛋白、白细胞介素-10水平降低,观察组低于对照组(P<0.05);治疗后,两组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平升高,观察组高于对照组(P<0.05)。结论:HA380血液灌流联合连续性肾脏替代治疗脓毒症合并AKI的效果显著,能够改善患者的血流动力学指标,降低炎性因子水平,提高免疫功能。
Objective:To explore the effect of HA380 hemoperfusion combined with continuous renal replacement therapy on sepsis complicated with acute kidney injury(AKI)and its influence on hemodynamics.Methods:A total of one hundred and two patients with sepsis complicated with AKI admitted to the Second Affiliated Hospital of Guizhou Medical University from January 2023 to February 2024 were selected as the study objects,and were divided into control group and observation group by random number table method,with fifty-one cases in each group.The control group received continuous renal replacement therapy,and the observation group was combined with HA380 hemoperfusion therapy based on the control group.The therapeutic effect,hemodynamic index level,inflammatory index level and T lymphocyte subsets were compared between the two groups.Results:The total effective rate of observation group was higher than that of control group(P=0.041);after treatment,the mean arterial pressure and central venous pressure of the two groups increased,and the heart rate decreased,the mean arterial pressure and central venous pressure of the observation group were higher than that of the control group,and the heart rate was lower than that of the control group(P<0.05);after treatment,the levels of interleukin-6,C-reactive protein and interleukin-10 in two groups were decreased,and the observation group was lower than the control group(P<0.05);after treatment,the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in both groups were increased,and the observation group was higher than the control group(P<0.05).Conclusion:HA380 hemoperfusion combined with continuous renal replacement has a significant effect in the treatment of sepsis complicated with AKI,which can improve the hemodynamic indexes of patients,reduce the levels of inflammatory factors,and improve the immune function.
