摘要
目的 探讨瘦素在慢性阻塞性肺部疾病 (COPD)患者体内的变化及意义。 方法 采用放射免疫法测定 3 4例稳定期COPD患者及 16例健康人的血清瘦素水平及肿瘤坏死因子 α(TNF α)水平 ,同时测定各营养指标 ,包括理想体重百分比 (IBW % )、三头肌皮褶厚度 (TSF)、上臂中点肌肉周径 (AMC)、肌酐身高指数 (CHI% )、总淋巴细胞计数 (TLC)及血清白蛋白 (ALB)。比较患者与健康人各参数的差异 ,瘦素与各参数的相关性。 结果 COPD患者各营养指标均明显低于健康人 (P <0 0 1) ,瘦素水平亦低于健康人 (P <0 0 5 ) ,TNF α水平高于健康人 (P <0 0 5 )。瘦素水平与各营养指标IBW % (r =0 69,P <0 0 1)、TSF(r =0 5 4,P <0 0 1)、AMC(r =0 5 2 ,P <0 0 1)、CHI% (r =0 5 8,P <0 0 1)呈显著正相关 ,与TNF α无明显相关性。 结论 COPD患者存在营养不良 ,瘦素水平低于健康人 。
Objective To explore the change of serum leptin and significance in patients with chronic obstructive pulmonary disease(COPD). Methods The levels of leptin and tumor necrosis factor-α (TNF-α) of 34 COPD patients in stationary phase and 16 healthy people were measured by radioimmunoassay. The ideal body weight percentage(IBW%), triceps skin-fold thickness(TSF), arm muscle circumference(AMC), creatinine height index percentage(CHI%), total lymphocyte(TLC), albumin(ALB) were determined at the same time. The differences of parameters between patients and healthy people were compared. The correlation between parameters and leptin levels was analysed. Results Nutritional parameters were lower in COPD patients than in healthy controls(P<0.01). The levels of leptin were lower than in healthy controls(P<0.05). The levels of TNF-α were increased than in healthy controls(P<0.05). Serum leptin levels were significantly correlated with IBW%?TSF?AMC?CHI%. Serum leptin levels did not correlate with TNF-αlevels. Conclusions Malnutrition occurred in patients with COPD. Levels of leptin were lower in patients with COPD than in the healthy controls. The physiologic regulation of leptin is maintained despite weight loss in patients with COPD.
