摘要
目的探讨血清同型半胱氨酸联合脂蛋白(a)检测对于冠状动脉粥样硬化疾病的预测价值及意义。方法选取80例2023年1月—10月莆田市第一医院收治的冠状动脉粥样硬化疾病患者为试验组,并随机选择80名同期来院体检的健康人群为对照组。检测患者血清同型半胱氨酸与脂蛋白(a)水平,分析两项指标单独检测与联合检测对于冠状动脉粥样硬化疾病检测阳性率及疾病诊断的特异度、敏感度、Youden指数和诊断符合率指标的差异。结果试验组HCY水平与Lp(a)水平均高于对照组L(均P<0.05)。试验组HCY、Lp(a)与HCY联合Lp(a)检测阳性率高于对照组(均P<0.05),且HCY联合Lp(a)检测阳性率最高。HCY联合Lp(a)检测的特异度、灵敏度、Youden指数与诊断符合率均高于单独HCY方法检测与单独Lp(a)方法检测(P<0.05)。结论血清HCY联合Lp(a)检测有助于提高冠状动脉粥样硬化疾病阳性率,较单独检测方式具有更高的诊断效能,对临床评估患者病情具有参考意义。
Objective To investigate the diagnostic value and significance of serum HCY combined with Lp(a)in coronary atherosclerosis.Methods A total of 80 patients with coronary atherosclerotic disease treated at the First Hospital of Putian from January 2023 to October 2023 were selected as the trial group,and a random selection of 80 healthy individuals who came to the hospital for physical examinations during the same period served as the control group.The levels of serum homocysteine and lipoprotein(a)were detected in the patients,and the differences in the positive rate of coronary atherosclerotic disease detection,specificity,sensitivity,Youden’s index,and diagnostic concordance rate between the two indicators when tested alone and in combination were analyzed.Results The levels of homocysteine(HCY)and lipoprotein(a)[Lp(a)]in the trial group were both higher than those in the control group(all P<0.05).The positive detection rates for HCY,Lp(a),and the combined detection of HCY and Lp(a)in the trial group were higher than those in the control group(all P<0.05),with the combined detection of HCY and Lp(a)showing the highest positive rate.The specificity,sensitivity,Youden’s index,and diagnostic concordance rate of the combined detection of HCY and Lp(a)were all higher than those of the detection methods using HCY alone and Lp(a)alone(P<0.05).Conclusions The combined detection of serum homocysteine(HCY)and lipoprotein(a)[Lp(a)]helps to increase the positive rate of coronary atherosclerotic disease and has a higher diagnostic efficacy compared to individual detection methods,which has reference significance for the clinical assessment of patients’conditions.
