摘要
目的:观察复方α-酮酸联合血液透析治疗肾衰竭患者的效果。方法:回顾性分析2021年5月至2023年2月该院收治的118例肾衰竭患者的临床资料,根据治疗方法不同将其分为对照组和研究组各59例。对照组采用血液透析治疗,研究组在对照组基础上联合复方α-酮酸治疗,比较两组治疗前及治疗3、6个月后钙磷代谢指标[血磷、血钙、全段甲状旁腺激素(iPTH)]、T细胞亚群指标(CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+))、血清学指标[β2-微球蛋白(β2-MG)、成纤维细胞生长因子(FGF)-23、结缔组织生长因子(CTGF)]水平,以及不良反应发生率。结果:治疗3、6个月后,两组血钙水平均高于治疗前,且研究组高于对照组,两组血磷、iPTH水平均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);两组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05);两组β2-MG、FGF-23、CTGF水平均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:复方α-酮酸联合血液透析治疗肾衰竭患者可提高T细胞亚群指标水平,改善钙磷代谢指标水平,以及降低血清学指标水平的效果优于单纯血液透析治疗。
Objective:To observe effects of Compoundα-keto acid combined with hemodialysis in treatment of patients with renal failure.Methods:The clinical data of 118 patients with renal failure admitted to this hospital from May 2021 to February 2023 were retrospectively analyzed.According to different treatment methods,they were divided into control group and study group,59 cases in each group.The control group was treated with hemodialysis,while the study group was treated with Compoundα-keto acid on the basis of that of the control group.The levels of calcium and phosphorus metabolism indexes[serum phosphorus,serum calcium,intact parathyroid hormone(iPTH)],T cell subsets indexes(CD3^(+),CD4^(+),CD4^(+)/CD8^(+))and serological indexes[β2-microglobulin(β2-MG),fibroblast growth factor(FGF)-23,connective tissue growth factor(CTGF)]before the treatment as well as 3 and 6 months after the treatment,and the incidence of adverse reactions were compared between the two groups.Results:After 3 and 6 months of treatment,the blood calcium levels of the two groups were higher than those before the treatment,and that in the study group was higher than that in the control group;the levels of serum phosphorus and iPTH in the two groups were lower than those before the treatment,and those in the study group were lower than those in the control group;and the differences were statistically significant(P<0.05).The levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the two groups were higher than those before the treatment,those in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).The levels ofβ2-MG,FGF-23 and CTGF in the two groups were lower than those before the treatment,those in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Compoundα-keto acid combined with hemodialysis in the treatment of the renal failure patients can improve the levels of T cell subsets,improve the levels of calcium and phosphorus metabolism indexes,and reduce the levels of serological indexes.Moreover,it is superior to simple hemodialysis.
