摘要
盆腔炎性不孕症属于临床常见病,中医治疗在增强机体免疫力、促进炎症吸收、改善盆腔环境等方面具有优势。本文从辨证思路、治疗特色、临证施治等方面系统阐述金志春教授治疗盆腔炎性不孕症的学术思想与经验。辨证思路提出本病属本虚标实之症,肾虚为本,血瘀为标,二者互为因果互促。引“邪之所凑、其气必虚”思想,断肾虚血瘀胞阻病因病机。治疗特色以“补肾活血、祛瘀通胞脉”为基本大法,因人制宜,灵活辨证,兼夹症状,顾阴阳气血平衡。辅以清热利湿、温阳暖宫、滋阴降火、疏肝调脾、气血双补等治法。衷中西合参,喜抗炎中药。临证施治承前人之学术思想,立足整体,谨守病机,补肾活血祛瘀,结合兼证行辨证论治,促进妊娠,为解决患者不孕症难题提供参考和思路。
Pelvic inflammatory infertility is a common disease in clinic.Traditional Chinese medicine(TCM)therapy has advantages in enhancing body immunity,promoting inflammatory absorption,and improving the pelvic environment,etc.This paper systematically elaborates on the academic thoughts and experience of professor Jin Zhichun in treating pelvic inflammatory infertility from the perspectives of dialectical thinking,treatment characteristics,and clinical application.In terms of dialectical thinking,it is proposed that the disease is a syndrome of deficiency in the root and excess in the branches,with kidney deficiency as the root and blood stasis as the branch,and the two are interdependent and mutually promoting as cause and result.The idea that"where pathogens gather,the energy must be deficient"is used to determine the pathogenesis of kidney deficiency and blood stasis obstructing the uterus.In terms of the treatment characteristics,the basic principle is to"tonify the kidney,activate blood circulation,remove stasis,and unblock the uterine vessels",together with adapting to the individual,flexibly applying dialectical treatment,addressing accompanying symptoms,and considering the balance of Yin,Yang,Qi,and blood.It is supplemented by methods such as clearing heat and resolving dampness,warming yang and warming the uterus,nourishing yin and reducing fire,soothing the liver and regulating the spleen,and tonifying both Qi and blood.The approach advocates for the integration of TCM and western medicine,with a preference for anti-inflammatory Chinese herbs.In clinical application,it inherits the academic thoughts of predecessors,focuses on the whole,strictly adheres to the pathogenesis,tonifies the kidney,activates blood circulation,and removes stasis,combining with the dialectical treatment of concurrent symptoms to promote pregnancy and provide references and ideas for solving the infertile problems of patients.
