摘要
呼吸机相关性肺炎(ventilator-associated pneumonia,VAP)是重症监护病房中常见的一种严重感染,与机械通气患者口腔内的微生物环境密切相关。氯己定作为一种广泛使用的口腔卫生消毒剂,可能在预防VAP方面起到一定作用,但近年来氯己定用于口腔护理出现一定争议。本文综述了氯己定的抗菌机理、对VAP预防的作用、不同浓度和频次的氯己定在VAP预防中的应用效果,以及安全性和细菌耐药性问题,以期为氯己定的临床口腔护理应用和相关研究提供参考。
Ventilator-associated pneumonia(VAP)is a common and serious infection in the intensive care unit(ICU),closely linked to the microbial environment in the mouth of patients with mechanical ventilation.Chlorhexidine,a widely used oral hygiene disinfectant,may play a role in preventing VAP.However,in recent years,there has been some controversy surrounding the use of chlorhexidine in oral care.This article summarizes the antibacterial mechanism of chlorhexidine,its role in preventing VAP,the effects of chlorhexidine at different concentrations and frequencies in VAP prevention,as well as safety and bacterial resistance issues,to provide reference for the clinical oral care application and related research of chlorhexidine.
