摘要
目的探究精浆弹性蛋白酶与精液参数的相关性。方法选取2022年3月至2023年12月深圳市妇幼保健院就诊的156例男性不育患者,根据精浆弹性蛋白酶水平将患者分为对照组和炎症组,对照组精浆弹性蛋白酶浓度<600 ng/ml 103例,炎症组精浆弹性蛋白酶浓度≥600 ng/ml 53例。根据世界卫生组织发布的第六版《人类精液检查与处理实验室手册》对所有患者精液进行一般性征、生化参数与形态学评估分析,以探讨弹性蛋白酶与精液参数之间的相关性。结果两组精液量、精子浓度、前向运动精子水平比较,差异有统计学意义(P<0.05)。两组精液白细胞计数、精浆弹性蛋白酶、精子顶体酶活性定量、精浆锌、精浆柠檬酸、精浆中性α-葡糖苷酶、支原体阳性率比较,差异有统计学意义(P<0.05)。两组精子缺陷总数、头部缺陷个数、颈部和中段的缺陷个数、尾部缺陷个数比较,差异有统计学意义(P<0.05)。精浆弹性蛋白酶水平与精液白细胞计数、支原体阳性率呈正相关(rs>0,P<0.05),精浆弹性蛋白酶水平与精液量、精子浓度、精子顶体酶水平、精浆锌水平、精浆柠檬酸水平、精浆中性α-葡糖苷酶水平呈负相关(r_(s)<0,P<0.05)。结论精浆弹性蛋白酶是提示生殖系感染的可靠指标,精液质量与精浆弹性蛋白酶密切相关,临床应加强对男性不育患者的弹性蛋白酶的检测,尽早采取干预措施,改善精液质量。
Objective To explore the correlation analysis of seminal plasmic elastase and semen parameters.Methods A total of 156 male infertility patients were selected from Shenzhen Maternal and Child Health Hospital from March 2022 to December 2023.Patients were divided into control group and inflammation group according to seminal plasmic elastase levels.The seminal plasma elastase concentration in control group was<600 ng/ml,103 patients,the concentration of seminal plasma elastase in the inflammatory group was≥600 ng/ml,53 patients.According to the sixth edition of the World Health Organization Laboratory Manual for Human Semen Inspection and Processing general sexual characteristics,biochemical parameters and morphological analysis were performed on semen of all patients,to investigate the correlation between elastase and semen parameters.Results Semen volume,sperm concentration,forward motility sperm level were compared between the two groups,the difference was statistically significant(P<0.05).Semen white blood cell count,seminal plasmic elastic proteinase,sperm acrosomal enzyme activity,seminal plasma zinc,seminal plasma citric acid,seminal plasma neutralα-glucosidase and mycoplasma positive rate were compared between the two groups,the difference was statistically significant(P<0.05).The total number of sperm defects,the number of head defects,the number of neck and middle defects,and the number of tail defects between the two groups,the difference was statistically significant(P<0.05).Serum elastase level was positively correlated with semen white blood cell count and mycoplasma positive rate(rs>0,P<0.05),there was a negative correlation between seminal volume,sperm concentration,acrosomal enzyme level,seminal zinc level,seminal citric acid level and seminal neutralα-glucosidase level(r_(s)<0,P<0.05).Conclusion Elastase is a reliable indicator of genital infection,and semen quality is closely related to seminal plasma elastase.In clinical practice,it is necessary to strengthen the detection of elastase in male infertility patients,and take early intervention measures to improve semen quality.
