肌内血管瘤的MRI征象及临床病理对照分析

摘要 目的探讨肌内血管瘤的MRI征象并与病理对照分析,以提高对该病的影像学认识。方法选取26例经手术和病理证实的肌内血管瘤临床和MRI资料,结合病理回顾性分析MRI征象。结果26例患者中A型2例(7.7%),B型6例(23.1%),C型18例(69.2%);T_(1)WI呈等或稍高信号,其内见蠕虫征6例(23.1%),索条征7例(27.0%),瘤内脂肪征22例(84.6%)包括花斑征10例(38.5%)和花环征12例(46.1%);T_(2)WI以高信号为主,瘤内蠕虫征16例(65.4%),索条征25例(96.0%)。瘤周鼠尾征T_(1)WI及T_(2)WI各显示17例(65.4%),瘤周脂肪征7例(27.0%);18例增强扫描的病灶均呈明显强化(均匀或不均匀)。结论肌内血管瘤成分复杂,MRI信号多样,瘤周和瘤内的征象对准确诊断和鉴别诊断肌内血管瘤有一定的帮助。 Objective To explore the MRI features of intramuscular hemangioma and to compare with the pathological analysis,so as to improve the imaging understanding of this disease.Methods Clinical and MRI data of 26 cases of intramuscular hemangiomas confirmed by surgery and pathology were collected,and MRI signs were retrospectively analyzed in conjunction with pathology.Results There were 2 cases of type A(7.7%),6 cases of type B(23.1%),and 18 cases of type C(69.2%).T1WI showed equal or slightly higher signal intensity,with worm sign in 6 cases(23.1%),cord sign in 7 cases(27.0%),and intratumoral fat sign in 22 cases(84.6%),including spotted sign in 10 cases(38.5%)and wreath sign in 12 cases(46.1%).T2WI mainly showed high signal intensity,with 16 cases(65.4%)showing intratumoral worm sign,and 25 cases(96.0%)showing cord sign.There were 17 cases(65.4%)of T_(1)WI and T_(2)WI with peritumoral mouse tail sign,and 7 cases(27.0%)with peritumoral fat sign.All of the 18 cases showed obvious enhancement(uniform or non-uniform).Conclusion Intramuscular hemangioma has complex components and varied MRI signals,the Peritumoral and intratumoral signs are helpful for accurate diagnosis and differential diagnosis of intramuscular hemangioma.
