摘要
冯志荣教授认为,风邪为荨麻疹的重要致病因素,秉承“治风必先治血,血行风自灭”之核心理念,提出“血瘀风动”理论。冯教授认为,在荨麻疹的病理演变过程中,“血瘀”和“风动”是两大重要致病因素,“风动”为标,“血瘀”为本。将荨麻疹的病机分为热灼血瘀、寒凝血瘀、血虚滞瘀,以“治血则内风熄,祛风则外风去”为治则。在治疗时,分别予以凉血、温经、养血之法,并以活血祛风法贯穿治疗始终。热盛血瘀证常用方剂为清营汤或消风散加减,寒凝血瘀证常用方剂为麻黄桂枝各半汤或当归四逆汤加减,血虚风燥证常用方剂为当归饮子加减。此外,冯教授还重视预防调护,在实际临床中,面对复杂多变的病情,秉持具体问题具体分析的严谨态度,充分考虑地域、时令、个体差异等因素,进行全面而细致的辨证施治。
Professor Feng Zhirong believes that wind pathogen is an important factor causing urticaria.Adhering to the core concept of"treating wind by treating blood first,and eliminating wind by flowing blood smoothly",he has proposed the theory of"blood stasis and wind movement".According to Professor Feng,in the pathological evolution of urticaria,"blood stasis"and"wind movement"are two major pathogenic factors."Wind movement"is the symptom,while"blood stasis"is the root cause.He classifies the pathogenesis of urticaria into three types:heat-scorching blood stasis,cold-coagulating blood stasis,and blood deficiency with stasis.The therapeutic principle is"to treat blood stasis for calming internal wind,and eliminate wind for removing external wind."Therefore,he applies methods of cooling blood,warming meridians,and nourishing blood respectively,and consistently employs the method of promoting blood circulation and eliminating wind throughout the treatment process.For syndrome of heat-excess blood stasis,the commonly used formulas are Ying Level Heat-Clearing Decoction or Modified Wind-Dispersing Powder.For syndrome of cold-coagulation and blood stasis,the commonly used formulas are Combined Ephedra Decoction and Cinnamon Twig Decoction in equal amounts or Modified Frigid Extremities Decoction.For syndrome of blood deficiency and wind dryness,the commonly used formula is Modified Chinese Angelica Drink.In addition,Professor Feng also attaches great importance to preventive care and health maintenance.In actual clinical practice,when confronted with complex and ever-changing conditions,he adheres to a rigorous attitude of analyzing specific problems on their own merits,fully considering factors such as geographical location,seasonal variations,and individual differences,to carry out comprehensive and meticulous diagnosis and treatment based on syndrome differentiation.
