摘要
目的:超声引导联合脑电双频谱指数(BIS)监测下竖脊肌平面阻滞(ESPB)复合全身麻醉应用于老年腰椎手术患者的效果。方法:选取2022年9月—2023年9月三明市第二医院接受腰椎手术的106例老年患者作为研究对象,采用随机数表法分为对照组和观察组,每组各53例。对照组给予气管插管下静吸复合全身麻醉干预,观察组给予超声引导联合BIS监测下ESPB复合全身麻醉干预。观察两组麻醉相关指标、血流动力学指标[心率(HR)、平均动脉压(MAP)]、术后疼痛程度[视觉模拟评分(VAS)]及不良反应发生情况。结果:观察组麻醉起效时间、拔管时间、苏醒时间短于对照组,差异有统计学意义(P<0.05)。两组术前血流动力学指标比较,差异无统计学意义(P>0.05);术中及术毕观察组HR、MAP水平明显低于对照组,差异有统计学意义(P<0.05)。术后6h、12h及24h,观察组VAS评分较对照组低,差异有统计学意义(P<0.05)。观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:对老年腰椎手术患者采用超声引导联合BIS监测下ESPB复合全身麻醉能够缩短麻醉起效和苏醒时间,维持血流动力稳定状态,降低术后疼痛程度及不良反应发生率。
Objective:The effect of ultrasound guidance combined with bispectral index(BIS)monitoring erector spinae plane block(ESPB)combined with general anesthesia in elderly patients with lumbar spine surgery.Method:A total of 106 elderly patients who received lumbar surgery in The Second Hospital of Sanming from September 2022 to September 2023 were selected as the study objects,and were divided into control group and observation group by random number table method,with 53 cases in each group.The control group was given general anesthesia under endotracheal intubation,while the observation group was given ultrasound guidance combined with BIS monitoring ESPB combined general anesthesia.Anesthesia related indexes,hemodynamic indexes[heart rate(HR),mean arterial pressure(MAP)],postoperative pain degree[visual analogue score(VAS)]and the occurrence of adverse reactions were observed between the two groups.Result:The effective time of anesthesia,extubation time and recovery time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the comparison of preoperative hemodynamic indexes between the two groups(P>0.05).HR and MAP levels in the observation group were significantly lower than those in the control group during and after the operation,and the difference was statistically significant(P<0.05).6 h,12 h and 24 h after operation,VAS scores in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion:Ultrasound-guided ESPB combined general anesthesia under BIS monitoring can shorten the onset and recovery time of anesthesia,maintain stable hemodynamic state,and reduce postoperative pain and incidence of adverse reactions in elderly patients undergoing lumbar surgery.
