摘要
目的:分析不同浓度轻比重罗哌卡因联合纳布啡椎管内阻滞用于剖宫产手术患者的效果及对血流动力学的影响。方法:选取2021年1月—2023年12月厦门市中医院82例接受剖宫产手术患者作为研究对象,遵循随机数字表法划分为对照组及观察组,每组41例。对照组给予0.5%罗哌卡因联合纳布啡椎管内阻滞、观察组给予0.375%罗哌卡因联合纳布啡椎管内阻滞,比较两组血流动力学水平、麻醉平面、不良反应发生率、术后疼痛评分。结果:观察组T_(2)、T_(3)时刻心率高于对照组,平均动脉压低于对照组(P<0.05);观察组不良反应发生率为2.44%,低于对照组的12.20%(P<0.05);两组术后12、24 h疼痛评分比较,差异无统计学意义(P>0.05);观察组运动阻滞、感觉平面阻滞方面的起效时间与对照组比较,差异无统计学意义(P>0.05),但观察组运动阻滞、感觉平面阻滞方面的维持时间短于对照组(P<0.05)。结论:0.375%轻比重罗哌卡因联合纳布啡椎管内阻滞在剖宫产手术中可提供有效的镇痛效果,改善血流动力学状况,降低不良反应发生率,改善麻醉平面的分布情况,很大程度上提高了麻醉安全性,在临床中具有借鉴及应用推广价值。
Objective:To analyze the effect of diferent concentrations of hypobaric ropivacaine combined with nalbuphine intraspinal block on patients undergoing cesarean section and its influence on hemodynamics.Methods:A total of 82 patients undergoing cesarean section in Xiamen Traditional Chinese Medicine Hospital from January 2021 to December 2023 were selected as the research objects.According to the random number table method,they were divided into control group and observation group,with 41 patients in each group.The control group was given 0.5%ropivacaine combined with nalbuphine intraspinal block,and the observation group was given 0.375%ropivacaine combined with nalbuphine intraspinal block.The hemodynamic level,anesthesia plane,incidence of adverse reactions,and postoperative pain score were compared between the two groups.Results:The heart rate at T_(2) and T_(3) in the observation group was higher than that in the control group,and the mean arterial pressure was lower than that in the control group(P<0.05).The incidence of adverse reactions in the observation group was 2.44%,which was lower than 12.20%in the control group(P<0.05).There was no significant diference in pain scores between the two groups at 12 and 24 h after operation(P>0.05).There was no significant difference in the onset time of motor block and sensory plane block between the observation group and the control group(P>0.05),but the maintenance time of motor block and sensory plane block in the observation group was shorter than that in the control group(P<0.05).Conclusion:0.375%hypobaric ropivacaine combined with nalbuphine intraspinal block can provide effective analgesic effect in cesarean section,improve hemodynamics,reduce the incidence of adverse reactions,improve the distribution of anesthesia plane,and greatly improve the safety of anesthesia.It has reference and application value in clinical practice.
