摘要
目的探究血清C反应蛋白(CRP)、血管内皮生长因子(VEGF)、细胞间黏附因子-1(ICAM-1)水平与老年糖尿病视网膜病变患者微血管损伤的相关性。方法选取2022年1月—2023年12月博山经济开发区卫生院收治的74例老年糖尿病视网膜病变患者,均接受眼底检查及眼底荧光血管造影检查,观察其微血管损伤情况,将其分为非增殖性视网膜病变组(非增殖组,44例)及增殖性视网膜病变组(增殖组,30例),所有患者均于入院时测定血清CRP、VEGF、ICAM-1水平,分析血清CRP、VEGF、ICAM-1水平与老年糖尿病视网膜病变患者微血管损伤的相关性。结果增殖组血清CRP、VEGF、ICAM-1水平均高于非增殖组,差异有统计学意义(P<0.05);多因素Logistic回归分析结果显示,高水平血清CRP、VEGF、ICAM-1是老年糖尿病视网膜病变患者微血管损伤的危险因素(P<0.05)。Spearman秩相关性检验结果显示,血清CRP、VEGF、ICAM-1水平与老年糖尿病视网膜病变患者微血管损伤呈正相关(r=0.405、0.357、0.423,均P<0.05)。结论血清CRP、VEGF、ICAM-1水平与老年糖尿病视网膜病变患者微血管损伤存在关联,呈现正相关。
Objective To explore the correlation between serum C reactive protein(CRP),vascular endothelial growth factor(VEGF),intercellular adhesion molecule-1(ICAM-1)levels and microvascular injury in elderly patients with diabetic retinopathy.Methods A total of 74 elderly patients with diabetic retinopathy admitted to Boshan Economic Development Zone Hospital from January 2022 to December 2023 were selected.All patients underwent fundus examination and fundus fluorescein angiography to observe the microvascular injury.They were divided into the non-proliferative retinopathy group(non-proliferative group,44 cases)and the proliferative retinopathy group(proliferative group,30 cases).Serum CRP,VEGF and ICAM-1 levels were measured in all patients on admission.The correlation between serum CRP,VEGF,ICAM-1 levels and microvascular injury in elderly patients with diabetic retinopathy was analyzed.Results The levels of serum CRP,VEGF and ICAM-1 in the proliferative group were higher than those in the non-proliferative group,and the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis results showed that high levels of serum CRP,VEGF and ICAM-1 were risk factors for microvascular injury in elderly patients with diabetic retinopathy(P<0.05).Spearman rank correlation test results showed that levels of serum CRP,VEGF and ICAM-1 were positively correlated with microvascular injury in elderly patients with diabetic retinopathy(r=0.405,0.357,0.423,all P<0.05).Conclusions The levels of serum CRP,VEGF and ICAM-1 are associated with microvascular injury in elderly patients with diabetic retinopathy,showing a positive correlation.
