摘要
目的探讨增液承气汤联合穴位埋线治疗孤独症谱系障碍(autism spectrum disorder,ASD)儿童功能性便秘(functional constipation,FC)的临床疗效。方法选取64例伴有FC的ASD患儿,采用随机数字表法分为对照组和研究组,每组32例。对照组给予常规治疗(包括教育和药物治疗),研究组在对照组基础上给予增液承气汤联合穴位埋线治疗。2组均连续治疗14 d。比较2组有效率、中医证候积分、排便情况、肠道菌群、胃肠激素等指标。结果治疗后,研究组有效率为93.75%(30/32),明显高于对照组的75.00%(24/32)(χ2=4.267,P=0.039)。2组主症积分、次症积分、排便时间、葡萄球菌数量、肠杆菌数量、生长抑素水平均低于治疗前(P均<0.05),且研究组上述指标均低于对照组(P均<0.05)。2组排便次数、Bristol粪便分型量表评分、双歧杆菌数量、P物质和胃动素水平均高于治疗前(P均<0.05),且研究组上述指标均高于对照组(P均<0.05)。结论增液承气汤联合穴位埋线治疗ASD儿童FC能降低中医证候积分,增加排便次数、双歧杆菌数量、P物质和胃动素水平,并缩短排便时间,减少葡萄球菌数量、肠杆菌数量及生长抑素水平,疗效确切。
Objective To explore the clinical efficacy of Zengye Chengqi decoction combined with acupoint embedding for the treatment of functional constipation(FC)in children with autism spectrum disorder(ASD).Methods Sixty-four children with ASD accompanied by FC were selected and randomly divided into the control group and the study group using a random number table method,with 32 cases in each group.The control group received conventional treatment(including oral education and drug treatment),while the study group received Zengye Chengqi decoction combined with acupoint embedding on the basis of the control group.Both groups were treated continuously for 14 days.Indicators such as the effective rate,TCM syndrome scores,bowel movements,gut microbiota and gastrointestinal hormones were compared in the two groups.Results After treatment,the effective rate of the study group was 93.75%(30/32),which was higher than the 75.00%(24/32)of the control group(χ2=4.267,P=0.039).The main symptom scores,secondary symptom scores,defecation time,Staphylococcus aureus counts,Escherichia coli counts and levels of somatostatin in both groups were lower than those before treatment(all P<0.05),and the above indicators in the study group were lower than those in the control group(all P<0.05).The frequency of defecation,Bristol fecal typing scale scores,Bifidobacteria counts,levels of substance P and motilin in both groups were higher than those before treatment(all P<0.05),and the above indicators in the study group were higher than those in the control group(all P<0.05).Conclusion The combination of Zengye Chengqi decoction and acupoint embedding therapy for FC in children with ASD could reduce traditional Chinese medicine syndrome scores,increase defecation frequency,Bifidobacteria counts,substance P and motilin levels,shorten defecation time,reduce Staphylococcus counts,Escherichia coli counts and somatostatin levels,which is with definite therapeutic effects.
