摘要
目的探索不同麻醉方式在超声引导下经外周静脉置入中心静脉导管(PICC)的应用效果。方法选取2021年9月至2022年7月安徽医科大学第二附属医院行超声引导下PICC置管的43例患者作为研究对象,按置管先后顺序分为对照组(23例)与试验组(20例)。对照组穿刺成功后在扩皮送鞘前使用利多卡因溶液进行局部注射麻醉,试验组在置管前使用利多卡因溶液湿敷,扩皮前予利多卡因溶液沿穿刺点滴入麻醉。比较两组患者在置管过程中的疼痛评分、满意情况及一次性置管成功率。结果在PICC置管过程中,试验组疼痛评分低于对照组,满意度高于对照组,差异有统计学意义(P<0.05)。两组患者的一次性置管成功率比较,差异无统计学意义(P>0.05)。结论PICC置管前使用利多卡因溶液湿敷,扩皮前予利多卡因溶液沿穿刺点滴入麻醉,可以有效降低患者的疼痛感,提高患者满意度。
Objective To explore the effectiveness of different anesthesia methods in ultrasound-guided peripherally inserted central venous catheter(PICC).Methods A total of 43 patients who underwent ultrasound-guided PICC placement in the Second Affiliated Hospital of Anhui Medical University from September 2021 to July 2022 were selected as the study objects,and they were divided into the control group(23 patients)and the experimental group(20 patients)according to the sequence of placement.The control group was given local injection anesthesia with Lidocaine solution before skin dilation and sheath delivery after successful puncture.In the experimental group,Lidocaine solution was used as a wet compress before tube placement,and Lidocaine solution was given to anesthetize along the puncture point before skin dilatation.The pain scores,satisfaction and one-time success rate of tube placement of the two groups of patients in the process of tube placement were compared.Results In the process of PICC cannulation,the pain scores of the experimental group were lower than those of the control group,and the satisfaction rate was higher than that of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the success rate of one-time tube placement between the two groups(P>0.05).Conclusion The use of Lidocaine solution wet compresses before PICC tube placement,and the anesthesia of Lidocaine solution dripping along the puncture point before skin expansion can effectively reduce the pain of patients and improve patient satisfaction.
