不同性别人群定量CT骨密度与血压和血脂的相关性分析

Correlation analysis of quantitative CT bone mineral density with blood pressure and blood lipid in different gender population

摘要 目的:探讨不同性别的健康体检人群,其骨密度与血压、血脂的相关性。方法:选取2018年-2020年在河南省人民医院健康管理科进行体检的4372名体检患者,分别测量其身高、体重、BMI、骨密度、血压、血脂、空腹血糖,使用Pearson相关或Spearman相关法分析其与骨密度的相关。结果:在男性体检患者中,骨密度与年龄、收缩压、高密度脂蛋白呈负相关,与BMI、舒张压、总胆固醇、低密度脂蛋白、甘油三酯呈正相关;女性患者骨密度与年龄、BMI、舒张压、收缩压、总胆固醇、低密度脂蛋白、甘油三酯呈负相关,与高密度脂蛋白没有相关性。进一步校正年龄、BMI后,男性患者骨密度与收缩压(β=-0.05,P=0.03)、甘油三酯(β=-0.07,P<0.01)、高密度脂蛋白(β=-0.10,P<0.01)呈负相关,与舒张压(β=0.06,P<0.01)、总胆固醇(β=0.14,P=0.03)呈正相关;女性患者骨密度与收缩压(β=-0.22,P<0.01)呈负相关,与舒张压呈正相关(β=0.08,P<0.01)。结论:对于不同性别的人群,维持生化指标在正常范围内,对于骨量减少具有重要的预警作用,对于骨质疏松患者,更应严格控制其血压、血脂水平,以改善患者的预后。 Objective:To investigate the correlation between bone mineral density(BMD)and blood pressure and lipids in healthy people of different genders undergoing health check-ups.Methods:A total of 4372 patients undergoing physical examinations in the Department of Health Management of Henan Provincial People's Hospital from 2018 to 2020 were selected.Their height,weight,BMI,bone mineral density,blood pressure,blood lipids and fasting blood glucose were measured.The correlation between these factors and bone mineral density was analyzed using Pearson correlation or Spearman correlation methods.Results:BMD was negatively correlated with age,systolic blood pressure,high density lipoprotein,and positively correlated with BMI,diastolic blood pressure,total cholesterol,low density lipoprotein and triglyceride in male physical examination patients.BMD was negatively correlated with age,BMI,diastolic blood pressure,systolic blood pressure,total cholesterol,low density lipoprotein,triglyceride,but not with high-density lipoprotein.After further adjustment for age,BMI,male patients with bone density and systolic blood pressure(β=-0.05,P=0.03),triglycerides(β=-0.07,P<0.01),high density lipoprotein(β=-0.10,P<0.01)showed a negative correlation.And diastolic blood pressure(β=0.06,P<0.01),total cholesterol(β=0.14,P=0.03)were positively correlated;In female patients,BMD was negatively correlated with systolic blood pressure(β=-0.22,P<0.01)and positively correlated with diastolic blood pressure(β=0.08,P<0.01).Conclusion:For people of different genders,maintaining biochemical indicators within the normal range plays an important role in warning against osteopenia.For patients with osteoporosis,it is necessary to strictly control their blood pressure and blood lipid levels to improve the prognosis of patients.
