摘要
睾丸扭转(testicular torsion,TT)是小儿外科常见的阴囊急症,一旦患儿疑似TT,临床上就需要及时准确地诊断、手法复位和手术治疗。TT的漏诊、误诊或者延迟诊断都会给患儿带来严重的后果,造成睾丸功能的损害或者缺失。目前关于TT对睾丸的损害理论、治疗方法以及预后的看法仍存在争议,本文将概述TT通过缺血再灌注和调节凝血损害同侧睾丸,通过自身免疫调节、交感神经介导的血管反射收缩以及原发性解剖变异等损害对侧睾丸,总结TT的临床表现和相应治疗策略。
Testicular torsion(TT)is a common scrotal emergency in pediatric surgery.Once suspected clinically,it needs to be timely and accurately diagnosed,and treated by manual reduction and surgery.Missed,misdiagnosed,or delayed diagnosis of TT can have serious consequences,such as impairment of testicular function or loss of testicles.At present,damage theory,treatment and prognosis of TT remain controversial.This review provided an overview of TT damage to the ipsilateral testis through ischemia-reperfusion and regulation of coagulation pathways,and damage to the contralateral testis through autoimmune regulation,sympathetic-mediated vasoreflex contraction,and primary anatomical variation,as well as clinical manifestations and corresponding strategies.
