摘要
目的探究25-羟维生素D[25(OH)D]对2型糖尿病性脑梗死患者认知功能及预后的影响。方法选取2021年8月至2024年1月我院收治的114例2型糖尿病性脑梗死患者为研究对象。根据血清25(OH)D水平及干预内容将所有患者分为阴性对照组(维生素D充足)、阳性对照组(维生素D不足)及干预组(维生素D不足,口服维生素D)。所有患者均接受基础治疗。分析2型糖尿病性脑梗死患者血清25(OH)D水平与生化指标的相关性;比较三组患者治疗前、后的神经功能、认知功能及预后。结果114例患者中,25(OH)D缺乏率为21.93%,不足率为43.86%,充足率为34.21%。血清25(OH)D水平与餐后2 h血糖呈负相关(r=-0.265,P=0.004)。治疗后,干预组的美国国立卫生研究院卒中量表(NIHSS)评分低于阴性对照组及阳性对照组(P<0.05)。治疗后,干预组的蒙特利尔认知评估量表(MoCA)评分高于阴性对照组及阳性对照组(P<0.05)。干预组的预后优于阴性对照组及阳性对照组(P<0.05)。结论对于低血清25(OH)D水平的2型糖尿病性脑梗死患者,补充维生素D可能是一种有效的干预措施,有助于改善其神经功能、认知功能及预后;在评估此类患者的病情时,建议常规检测血清25(OH)D水平。
Objective To explore the effects of 25-hydroxy vitamin D[25(OH)D]on cognitive function and prognosis in type 2 diabetes mellitus patients with cerebral infarction.Methods A total of 114 type 2 diabetes mellitus patients with cerebral infarction admitted in our hospital from August 2021 to January 2024 were selected as the study objects.According to serum 25(OH)D level and intervention content,the patients were divided into negative control group(sufficient vitamin D),positive control group(insufficient vitamin D)and intervention group(insufficient vitamin D,oral vitamin D).All patients received basic treatment.The correlation between serum 25(OH)D level and biochemical indexes in type 2 diabetes mellitus patients with cerebral infarction was analyzed;the neurological function,cognitive function before and after treatment and prognosis of the three groups were compared.Results Among the 114 patients,the 25(OH)D deficiency rate was 21.93%,the insufficiency rate was 43.86%,and the adequacy rate was 34.21%.Serum 25(OH)D level was negatively correlated with 2 h postprandial blood glucose(r=-0.265,P=0.004).After treatment,the National Institute of Health Stroke Scale(NIHSS)score of the intervention group was lower than that of the negative control group and the positive control group(P<0.05).After treatment,the Montreal Cognitive Assessment(MoCA)score of the intervention group was higher than that of the negative control group and the positive control group(P<0.05).The prognosis of the intervention group was better than that of the negative control group and the positive control group(P<0.05).Conclusion For type 2 diabetes mellitus patients with cerebral infarction with low serum 25(OH)D level,vitamin D supplementation may be an effective intervention to improve their neurological function,cognitive function and prognosis;when evaluating the condition of such patients,it is recommended to routinely detect serum 25(OH)D level.
