摘要
目的探讨腹腔镜下腹膜前腹股沟疝修补术后发生补片感染的影响因素。方法回顾性分析2022年1月—2023年12月于本院开展腹腔镜下腹膜前腹股沟疝修补术的600例患者的基本资料。对患者进行为期1年的随访,依据患者是否出现补片感染将其分为补片感染组(n=12)和未感染组(n=588)。对两组一般资料予以统计,在单因素和Logistic回归模型中作多因素分析,以明确影响腹腔镜下腹膜前腹股沟疝修补术后发生补片感染的独立因素,用ROC曲线评估诊断效能。结果单因素分析结果显示,两组在年龄、BMI指数、是否有糖尿病史、手术时间、出血量、住院时间、术后是否出现外科手术部位感染方面比较,差异有统计学意义(P<0.05);Logistic回归模型多因素分析结果显示,BMI指数(P=0.039,OR=1.232)、是否有糖尿病史(P=0.036,OR=1.624)、手术时间(P=0.000,OR=1.115)、术后是否出现外科手术部位感染(P=0.002,OR=2.939)是腹腔镜下腹膜前腹股沟疝修补术后补片感染的独立影响因素;由ROC曲线分析,BMI指数、是否有糖尿病史、手术时间、术后是否出现外科手术部位感染联合检测的AUC值最高,为0.984,且灵敏度、特异度分别为95.31%、91.67%。结论BMI指数、是否有糖尿病史、手术时间、术后是否出现外科手术部位感染是腹腔镜下腹膜前腹股沟疝修补术后补片感染的独立影响因素。在临床上应对相关因素予以有效控制,并施以有针对性的防治措施来降低腹腔镜下腹膜前腹股沟疝修补术后发生补片感染的风险。
Objective To explore influencing factors of prosthetic infection after laparoscopic anterior inguinal hernia repair.Methods The paper reviewed and analyzed basic data of 600 patients with laparoscopic anterior inguinal hernia repair in our hospital from January 2022 to December 2023,and divided them into prosthetic infection group(n=12)and non infection group(n=588)based on prosthetic infection situation after followup of one year.Statistical analysis of general data was carried out between two groups with univariate and logistic regression models to identify independent factors of prosthetic infection after laparoscopic anterior inguinal hernia repair.Diagnostic efficacy was evaluated with ROC curve.Results During univariate analysis,there was significant difference between two groups in age,BMI index,history of diabetes,operation time,blood loss,lenth of stay,and surgical infection after operation(P<0.05).Logistic regression model multifactor analysis showed,BMI index(P=0.039,OR=1.232),history of diabetes(P=0.036,OR=1.624),operation time(P=0.000,OR=1.115),surgical infection(P=0.002,OR=2.939)after laparoscopic anterior inguinal hernia repair were independent influencing factors of prosthetic infection.According to ROC curve analysis,AUC value of BMI index,history of diabetes,operation time,and surgical infection after operation was the highest of 0.984,sensitivity and specificity was 95.31%and 91.67%respectively.Conclusion BMI index,history of diabetes,operation time,and postoperative surgical infection are independent influencing factors of prosthetic infection of laparoscopic anterior inguinal hernia repair surgery.It is necessary to control relevant factors effectively and take targeted prevention and treatment measures in clinical practice to reduce risks of prosthetic infection after laparoscopic anterior inguinal hernia repair surgery.
