摘要
目的构建精细化护理策略并分析其在糖尿病肛周脓肿并发瘘管患者中的应用效果。方法选取南华大学附属第二医院于2021年3月至2023年3月收治的100例糖尿病肛周脓肿并发瘘管患者作为研究对象,采用随机数字表法分为观察组和对照组,每组50例。对照组接受常规护理,观察组接受精细化护理。比较2组的临床指标、炎症因子、疼痛程度、日常生活能力及并发症发生率。结果观察组的创面愈合和脓肿消退时间均较对照组更早,住院时间较对照组更短,并发症总发生率较对照组更低(P均<0.05)。术后3 d和出院前,观察组的血清白介素-6和肿瘤坏死因子-α水平、数字疼痛评分法评分均低于同期对照组,日常生活能力量表评分均高于同期对照组(P均<0.05),且2组均优于术前(P均<0.05);出院前,2组的上述指标和评分均优于术后3 d(P均<0.05)。结论构建并实施精细化护理策略有助于缩短糖尿病肛周脓肿并发瘘管患者的住院时间和症状消失时间,改善炎症因子水平和术后疼痛程度,降低并发症发生率,提高日常生活能力。
Objective To construct a refined nursing strategy and analyze its application effect in diabetic patients with perianal abscess complicated with fistula.Methods One hundred diabetic patients with perianal abscess complicated with fistula who were admitted to the Second Hospital,University of South China from March 2021 to March 2023 were selected as the research objects.They were divided into an observation group and a control group through a random number table,each with 50 cases.The control group received routine nursing,while the observation group received refined nursing.The clinical indicators,inflammatory factors,pain intensity,activities of daily living,and incidence of complications were compared between the two groups.Results The wound healing time and abscess healing time of the observation group were earlier than those of the control group.The hospitalization time was shorter than that of the control group.The overall incidence of complications was lower than that of the control group(all P<0.05).At 3 days after surgery and before discharge,the serum levels of interleukin-6 and tumor necrosis factor-α,as well as the Numerical Rating Scale scores in the observation group were lower than those in the control group,and the Activities of Daily Living Scale scores were higher than those in the control group(all P<0.05).Both groups were better than those before surgery(all P<0.05).Before discharge,the above indicators and scores of both groups were better than those at 3 days after surgery(all P<0.05).Conclusion The construction and implementation of refined nursing strategy can help shorten the hospital stay and symptom disappearance time of diabetic patients with perianal abscess complicated with fistula,improve the level of inflammatory factors and postoperative pain,reduce the incidence of complications,and improve the activities of daily living.
