无创正压通气用于低温射频扁桃体腺样体切除术治疗儿童重度OSA中的疗效评估

Evaluation on the Curative Effect of Low-temperature Radio Frequency Tonsil Adenoidectomy with Non-invasive Positive Pressure Ventilation in Children with Severe OSA

摘要 目的:评估无创正压通气用于低温射频扁桃体腺样体切除术治疗儿童重度阻塞性睡眠呼吸暂停(OSA)的疗效。方法:选择2021年1月—2023年1月河南省儿童医院收治的38例重度OSA患儿作为研究对象,按随机数表法分成对照组和观察组,每组各19例。对照组行低温射频扁桃体腺样体切除术,观察组行低温射频扁桃体腺样体切除术治疗时给予无创正压通气。对比两组患儿生活质量[OSA疾病特异性生活质量调查表-18 (OSA-18)]、多导睡眠图相关指标[最低血氧饱和度(LSaO_(2))、呼吸暂停低通气指数(AHI)]、疗效、术后并发症。结果:术后6个月,两组患儿OSA-18评分低于术前,且观察组低于对照组,差异有统计学意义(t=4.12,P<0.05);术后6个月,观察组LSaO2高于对照组,AHI低于对照组,差异均有统计学意义(t=3.327、5.449,P<0.05);术后6个月,两组患儿治疗总有效率比较,差异无统计学意义(χ^(2)=0.362,P>0.05);两组患儿术后并发症发生率比较,差异无统计学意义(χ^(2)=0.362,P>0.05)。结论:无创正压通气用于低温射频扁桃体腺样体切除术治疗儿童重度OSA中可有效改善LSaO_(2)和AHI指标,提高生活质量。 Objective:To evaluate the curative effect of low-temperature radiofrequency tonsil adenoidectomy with non-invasive positive pressure ventilation in children with severe obstructive sleep apnea(OSA).Methods:38 children with severe OSA admitted to the hospital were enrolled from January 2021 and January 2023.According to random number table method,they were divided into control group and observation group,with 19 children in each group.The control group was treated with low-temperature radiofrequency tonsil adenoidectomy,while observation group was additionally treated with non-invasive positive pressure ventilation.The quality of life[disease specific quality of life for children with obstructive sleep apnea 18 items survey(OSA-18)],polysomnography related indexes[the minimum oxygen saturation(LSaO_(2)),apnea hypopnea index(AHI)],curative effect and postoperative complications were compared between the two groups.Results:6 months after operation,the OSA-18 scores of the two groups were lower than those before operation(t=4.12,P<0.05),and the observation group was lower than the control group.6 months after operation,LSaO_(2) in observation group was higher than that in control group,and AHI was lower than that in control group,with statistically significant difference(t=3.327,5.449;P<0.05).6 months after surgery,there was no significant difference in the total effective rate between the two groups(χ^(2)=0.362,P>0.05).There was no significant difference in the incidence of postoperative complications between the two groups(χ^(2)=0.362,P>0.05).Conclusion:Low-temperature radiofrequency tonsil adenoidectomy combined with non-invasive positive pressure ventilation can effectively improve LSaO_(2) and AHI,and improve quality of life in children with severe OSA.
