摘要
目的分析在颈椎骨折伴高位截瘫患者中开展个体化护理的效果,探究其对压疮、下肢深静脉血栓形成(DVT)的预防作用。方法选取2019年5月—2023年7月本院收治的30例颈椎骨折伴高位截瘫患者作为研究对象,将其分为两组,每组15例。对照组行常规护理,实验组行个体化护理,比较不同护理模式的效果。结果干预前,两组下肢股静脉血流速度、下肢股静脉内径、Caprini评分对比,差异无统计学意义(P>0.05);干预后,实验组下肢股静脉血流速度、下肢股静脉内径大于对照组,Caprini评分低于对照组(P<0.05)。实验组患者6个月内DVT发率低于对照组(P<0.05)。两组患者6个月内肺栓塞发生率对比,差异无统计学意义(P>0.05)。实验组患者6个月内压疮发生率低于对照组(P<0.05)。结论在颈椎骨折伴高位截瘫患者中开展个体化护理,对压疮、DVT均有明显的预防作用。
Objective To analyze the effect of individualized nursing in patients with cervical spine fractures and high paraplegia,and to explore its preventive effect on pressure ulcers and lower limb deep vein thrombosis(DVT).Methods Thirty patients with cervical spine fractures and high paraplegia admitted to our hospital from May 2019 to July 2023 were selected as the research subjects and divided into two groups,with 15 cases in each group.The control group received routine care,and the experimental group received individualized care to compare the effects of different care models.Results Before the intervention,there was no statistically significant difference in the blood flow velocity of the lower limb femoral vein,the inner diameter of the lower limb femoral vein,and the Caprini score between the two groups(P>0.05);after the intervention,the blood flow velocity of the lower limb femoral vein,the lower limb femoral vein diameter,and the Caprini score of the two experimental groups were The inner diameter was larger than that of the control group,and the Caprini score of the experimental group was lower than that of the control group(P<0.05).The incidence rate of DVT in the experimental group and the control group within 6 months was higher than that in the control group,that is,6.67%lower than 40.00%,and the difference was significant(P<0.05).There was no statistical difference in the incidence of pulmonary embolism between the two groups of patients within 6 months(P>0.05).The incidence of pressure ulcers in the experimental group within 6 months was compared with the control group,that is,13.33%vs.46.67%,with a significant difference(P<0.05).Conclusion Carrying out individualized care in patients with cervical spine fractures and high paraplegia has a significant preventive effect on pressure ulcers and DVT.
