摘要
目的:分析美罗培南联合经鼻高流量湿化氧疗对重症肺炎患者的治疗效果。方法:选取2020年1月-2024年9月铜仁市人民医院收治的80例重症肺炎患者,按不同治疗方案分为两组。对照组(n=35)予以常规治疗及经鼻高流量湿化氧疗,观察组(n=45)在对照组基础上采用美罗培南治疗。对比两组治疗效果、临床症状体征缓解时间、呼吸功能、氧化应激反应、炎症因子。结果:观察组总有效率(97.78%)较对照组(80.00%)高,各症状体征缓解时间均较对照组早,差异均有统计学意义(P<0.05)。观察组治疗后呼吸功(WOB)较对照组低,动态顺应性(Cdyn)、氧合指数均较对照组高,差异均有统计学意义(P<0.05)。观察组治疗后超氧化物歧化酶(SOD)较对照组高,脂质过氧化(LPO)、炎症因子水平均较对照组低,差异均有统计学意义(P<0.05)。结论:美罗培南联合经鼻高流量湿化氧疗可提高重症肺炎治疗效果,改善患者呼吸功能,减轻氧化应激反应和炎症反应。
Objective:To analyze the therapeutic effect of Meropenem combined with high flow nasal humidified therapy on patients with severe pneumonia.Method:A total of 80 patients with severe pneumonia admitted to Tongren People's Hospital from January 2020 to September 2024 were selected and divided into two groups according to different treatment plans.The control group(n=35)received conventional treatment and high flow nasal humidified therapy,and the observation group(n=45)was treated with Meropenem on the basis of the control group.The therapeutic effect,clinical symptoms and signs relief time,respiratory function,oxidative stress response and inflammatory factors were compared between the two groups.Result:The total effective rate of the observation group(97.78%)was higher than that of the control group(80.00%),and the relief time of symptoms and signs were earlier than those of the control group,the differences were statistically significant(P<0.05).After treatment,the work of breathing(WOB)of the observation group was lower than that of the control group,and the dynamic compliance(Cdyn)and oxygenation index were higher than those of the control group,the differences were statistically significant(P<0.05).After treatment,the superoxide dismutase(SOD)in the observation group was higher than that in the control group,and the levels of lipid peroxidation(LPO)and inflammatory factors were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Meropenem combined with high flow nasal humidified therapy can improve the therapeutic effect of severe pneumonia,improve the respiratory function of patients and reduce oxidative stress response and inflammatory response.
