摘要
目的:观察达格列净联合二甲双胍治疗2型糖尿病(T2DM)患者的效果。方法:选取2021—2023年该院收治的78例T2DM患者进行前瞻性研究,按照随机数字表法将其分为对照组与观察组各39例。对照组给予二甲双胍治疗,观察组在对照组基础上联用达格列净治疗。比较两组临床疗效,治疗前后血糖指标[空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1c)]、胰岛功能指标[胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)]、血清炎性因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]水平,以及不良反应发生率。结果:观察组治疗总有效率为94.87%(37/39),高于对照组的79.49%(31/39),差异有统计学意义(P<0.05);治疗后,两组FPG、2hPG、HbA1c水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组HOMA-IR水平均低于治疗前,且观察组低于对照组,两组HOMA-β水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);治疗后,两组IL-6、TNF-α水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:达格列净联合二甲双胍治疗T2DM患者可提高治疗总有效率,改善胰岛功能指标水平,以及降低血糖指标和血清炎性因子水平,效果优于单纯二甲双胍治疗。
Objective:To observe effects of Dapagliflozin combined with Metformin in treatment of patients with type 2 diabetes mellitus(T2DM).Methods:A prospective study was conducted on 78 patients with T2DM admitted to this hospital from 2021 to 2023.According to the random number table method,they were divided into control group and observation group,39 cases in each group.The control group was treated with Metformin hydrochloride,while the observation group was treated with Dapagliflozin on the basis of that of the control group.The clinical efficacy,the levels of blood glucose indexes[fasting blood glucose(FPG),2-hour postprandial blood glucose(2hPG),glycosylated hemoglobin(HbA1c)],islet function[insulin resistance index(HOMA-IR),isletβcell function index(HOMA-β)]and serum inflammatory factors[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)]before and after the treatment,and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 94.87%(37/39),which was higher than 79.49%(31/39)of the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of FPG,2hPG and HbA1c in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the HOMA-IR levels of the two groups were lower than those before the treatment,and that in the observation group was lower than that in the control group;the levels of HOMA-βin the two groups were higher than those before the treatment,and that in the observation group was higher than that in the control group;and the differences were statistically significant(P<0.05).After the treatment,the levels of IL-6 and TNF-αin the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Dapagliflozin combined with Metformin in the treatment of the T2DM patients can improve the total effective rate of treatment,improve the islet function,and reduce the levels of blood glucose indexes and serum inflammatory factors.Moreover,it is superior to simple Metformin treatment.
