摘要
目的分析2016—2023年南充市脑及中枢神经系统恶性肿瘤发病、死亡和疾病负担的变化趋势,为脑及中枢神经系统恶性肿瘤的科学防治提供依据。方法根据2016—2023年中国肿瘤登记平台脑及中枢神经系统恶性肿瘤发病与死亡数据,使用Excel 2016、SPSS 23.0、Joinpoint Regression Programme 4.5.0.1软件进行数据整理和分析,计算发病率和死亡率及其标化率、早死损失寿命年(YLL)、伤残损失寿命年(YLD)和伤残调整寿命年(DALY),采用年度变化百分比(APC)分析脑及中枢神经系统恶性肿瘤发病率、死亡率及DALY率的变化趋势。检验水准α=0.05。结果2016—2023年南充市脑及中枢神经系统恶性肿瘤粗发病率、中标发病率、世标发病率、粗死亡率、中标死亡率、世标死亡率和DALY率分别为4.59/10万、2.84/10万、2.78/10万、3.25/10万、1.90/10万、1.86/10万和99.11/10万人年,8年间发病率、死亡率和DALY率均上升,发病率上升趋势无统计学意义(t=1.979,P=0.095),死亡率和DALY率上升趋势有统计学意义(t=4.876、3.781,P均<0.05)。脑及中枢神经系统恶性肿瘤发病率、死亡率和DALY率均随年龄的增长而升高。男性和女性脑及中枢神经系统恶性肿瘤DALY率均上升,趋势变化有统计学意义(t=3.218、3.199,P均<0.05)。结论2016—2023年南充市脑及中枢神经系统恶性肿瘤发病率相对稳定,死亡率和DALY率升高,疾病负担持续增加,尤其是男性及老年人群脑及中枢神经系统恶性肿瘤的疾病负担重。
Nanchong Hospital,Beijing Anzhen Hospital Affiliated to Capital Medical University/Nanchong Central Hospital;Nanchong Center for Disease Control and Prevention;Objective To analyze the changing trends of incidence,mortality and disease burden of malignant tumors of brain and central nervous system in Nanchong City from 2016 to 2023,and to provide basis for the scientific prevention and treatment of malignant tumors of brain and central nervous system.Methods According to the incidence and death data of malignant tumors of brain and central nervous system reported in the China Cancer Registration Platform from 2016 to 2023,Excel 2016,SPSS 23.0 and Joinpoint Regression Programme 4.5.0.1 software were used to organize and analyze the data.The incidence rate and mortality rate and their standardized rates,years of life lost(YLL),years lived with disability(YLD)and disability-adjusted life years(DALY)were calculated.The annual percentage change(APC)was used to analyze the changing trends of incidence rate,mortality rate and the DALY rate of malignant tumors of brain and central nervous system.The test level wasα=0.05.Results From 2016 to 2023,the crude incidence rate,the standardized incidence rate of the Chinese population,the standardized incidence rate of the world population,the crude mortality rate,the standardized mortality rate of the Chinese population,the standardized mortality rate of the world population,and the DALY rate of malignant tumors of brain and central nervous system in Nanchong City were 4.59/105,2.84/105,2.78/105,3.25/105,1.90/105,1.86/105and 99.11/105person years,respectively.The incidence,mortality and DALY rates all increased in the past eight years.There was no statistical significance in the upward trend of the incidence rate(t=1.979,P=0.095),but there were statistical significances in the upward trend of the mortality and DALY rates(t=4.876 and 3.781,all P<0.05).The incidence,mortality and DALY rates of malignant tumors of brain and central nervous system increased with the increase of age.The DALY rate of malignant tumors of brain and central nervous system increased in both males and females,and the trend was statistically significant(t=3.218 and 3.199,both P<0.05).Conclusions From2016 to 2023,the incidence rate of malignant tumors of brain and central nervous system in Nanchong City was relatively stable,the mortality rate and the DALY rate increased,and the disease burden continued to increase,especially the disease burden of malignant tumors of brain and central nervous system was heavy in men and the elderly.
