摘要
非酒精性脂肪肝病(NAFLD)是以肝细胞内脂质过度沉积及脂肪变性为主要特征的代谢性疾病,已成为全球最常见的慢性肝病。目前NAFLD发病机制尚未完全阐明,研究发现线粒体功能障碍在NAFLD的发展过程中起到重要作用。张介宾认为阳动而散,故化气,阴静而凝,故成形,基于此可以认为“阳化气不及、阴成形太过”是NAFLD的主要病机。因此,本文基于线粒体功能障碍,从理论依据、发病机制、病理生理联系以及现代研究等方面阐述“阳化气、阴成形”理论在NAFLD发病的科学内涵。深入微观层面探讨运用“阳化气、阴成形”理论指导NAFLD的治疗,以恢复线粒体功能(阳气或气),消除肝脏内生产物(阴邪),为本病的理论研究及临床防治提供参考。
Non-alcoholic fatty liver disease(NAFLD)is a metabolic disorder characterized by excessive lipid accumulation and fatty degeneration in hepatocytes,and has become the most common chronic liver disease worldwide.The pathogenesis of NAFLD is not yet fully understood,but studies have found that mitochondrial dysfunction plays a critical role in the progression of NAFLD.ZHANG Jiebin believed that yang disperses and transforms into qi,while yin is still and constitutes form.Based on this,it can be inferred that“insufficient transformation of qi by yang,and excessive forming by yin”is the main pathogenesis of NAFLD.This article explores the scientific connotation of the“yang transforming qi and yin constituting form”theory in the development of NAFLD,based on mitochondrial dysfunction,from the perspectives of theoretical foundation,pathogenesis,pathophysiological connections,and modern research.It delves into the microscopic level of how the theory of“yang transforming qi and yin constituting form”can guide the treatment of NAFLD,aiming to restore mitochondrial function(yang or qi)and eliminate hepatic production(yin evil),providing a reference for theoretical research and clinical prevention and treatment of the disease.
