摘要
偏瘫是中风后常见的一种后遗症,根据相关调查统计在中风患者中约有40%~50%的人群伴有不同程度的偏瘫,影响患者的预后康复效果。西医对于脑卒中后偏瘫治疗主要以药物进行脑保护,营养神经,促进脑损伤修复,以及抗血小板、调血脂等基础治疗,存在治疗周期长、起效慢、效果不明显等问题。中医认为该病属于“瘫证”“痿证”范畴,通过病机剖析和辨证论治,采取相应的中医治疗方法,能够显著提高脑卒中后偏瘫患者的治疗效果。针灸疗法作为中医经典治法,通过辨证取穴施针,能够起到疏经通络、祛瘀活血、开窍醒脑的功效,对于脑卒中后偏瘫有良好治疗效果。近年来,针灸疗法治疗脑卒中后偏瘫效果不断受到临床重视和认可,为该病的治疗提供了新的选择。通过查阅大量的国内外相关报道,对针灸疗法在脑卒中后偏瘫患者中的应用现状及进展进行综述,以期为脑卒中偏瘫患者的临床治疗提供参考。
Hemiplegia is a common sequela after stroke.According to relevant surveys and statistics,about 40%to 50%of stroke patients have varying degrees of hemiplegia,which affects the prognosis and rehabilitation effects of patients.Western medicine mainly uses drugs for brain protection treatment of post-stroke hemiplegia,nourishing nerves,promoting brain injury repair,as well as basic treatments such as antiplatelet and lipid-lowering.However,there are problems such as long treatment cycles,slow onset,and unclear effects.Traditional Chinese medicine believes that this disease belongs to the categories of"paralysis syndrome"and"impotence syndrome".By analyzing the pathogenesis and treating it based on syndrome differentiation,adopting corresponding traditional Chinese medicine treatment methods can significantly improve the treatment effect of post-stroke hemiplegia patients.Acupuncture and moxibustion,as a classic treatment of traditional Chinese medicine,can dredge channels and collaterals,remove blood stasis and activate blood circulation,and open the orifices and awaken the brain by applying acupuncture at acupoints based on syndrome differentiation,which has a good therapeutic effect on post-stroke hemiplegia.In recent years,the effect of acupuncture and moxibustion therapy on poststroke hemiplegia has been continuously valued and recognized by the clinic,providing a new choice for the treatment of this disease.This article reviews the application status and progress of acupuncture and moxibustion therapy in stroke patients with hemiplegia by consulting a large number of relevant reports at home and abroad,in order to provide reference for clinical treatment of stroke patients with hemiplegia.
