摘要
糖尿病心肌病是发生在糖尿病糖脂代谢紊乱及微血管病变基础上的一种心血管疾病,是糖尿病患者发生心力衰竭和死亡的主要原因之一。西医治疗该病尚无特效药,以控制血糖和缓解症状为主,伴随的不良反应限制了临床使用。中医药治疗该病经验丰富,临床疗效确切,用药安全性高。中药黄芪作为补益药,具有补气健脾、升阳举陷、益卫固表、利水消肿、托毒生肌之效。药理学研究表明其具有抗氧化、抗炎、降压、降糖调脂、调节免疫、抗肿瘤、保肝利胆、扩张外周血管、改善微循环等作用。黄芪治疗糖尿病心肌病的主要活性成分是黄芪多糖和黄芪甲苷,目前研究表明黄芪有效成分、黄芪药对、黄芪复方治疗糖尿病心肌病疗效显著。近年来研究发现,黄芪能够通过多靶点、多通路减轻氧化应激反应、抑制心肌细胞凋亡、抗心肌纤维化、调节能量代谢,进一步改善患者心肌炎症和纤维化、心肌损伤和糖脂代谢异常。作者旨在梳理相关文献,从黄芪有效成分、黄芪药对、黄芪复方3个方面归纳总结中药黄芪防治糖尿病心肌病的作用机制及研究进展。
Diabetic cardiomyopathy is a cardiovascular disease based on diabetes mellitus glucoselipid metabolism disorder and microangiopathy,and is one of the main causes of heart failure and death in diabetic patients.Western medicine has no effective drugs to treat this disease,but to control blood glucose and alleviate the symptoms of the main,accompanied by adverse reactions and side effects to limit the clinical use.Traditional Chinese medicine(TCM)has rich experience in treating this disease,with precise clinical efficacy and high safety.As a tonic medicine,Huangqi(Astragali Radix)has the effects of tonifying Qi and strengthening spleen,elevating Yang and lifting prolapsed Zang-Fu organs,benefiting Wei and consolidating epidermis,inducing diuresis and eliminating oedema,and supporting toxins to produce muscles.Pharmacological studies have shown that it has antioxidant,anti-inflammatory,antihypertensive,hypoglycemic and lipid-regulating,immune-regulating,anti-tumour,hepatoprotective and choleretic,peripheral vasodilator,and microcirculation improvement effects.The main active ingredients of Huangqi(Astragali Radix)in the treatment of diabetic cardiomyopathy are astragalus polysaccharide and astragalus methyl glycoside,and the current research shows that the active ingredients of Huangqi(Astragali Radix),Huangqi medicinal pairs,and Huangqi compound treatment of diabetic cardiomyopathy,the therapeutic efficacy is remarkable.In recent years,it has been found that Huangqi(Astragali Radix)is able to alleviate oxidative stress,inhibit cardiomyocyte apoptosis,anti-myocardial fibrosis,and regulate energy metabolism through multi-targets and multi-pathways,which further improves the patients'myocardial inflammation and fibrosis,myocardial injury,and abnormalities of glucose and lipid metabolism.The authors aimed to sort out the relevant literature and summarise the mechanism of action and research progress of the Chinese medicine Huangqi(Astragali Radix)in preventing and treating diabetic cardiomyopathy from three aspects,namely,the active ingredient of Huangqi(Astragali Radix),Huangqi medicinal pairs,and Huangqi compound.
