儿童系统性红斑狼疮外周血TB细胞亚群分析与疾病活动相关性研究

Study on correlation between peripheral blood TB cell subsets and disease activity in children with systemic lupus erythematosus

摘要 目的分析系统性红斑狼疮(SLE)患儿与健康儿童外周血TB细胞亚群的差异,并探讨TB细胞亚群与狼疮疾病活动的相关性。方法采用流式细胞术检测129名SLE患儿及195名健康儿童的外周血TB细胞亚群,按照性别及年龄行1∶1匹配,共匹配成功69对儿童,比较两组儿童TB细胞亚群的差异。SLE患儿根据疾病活动指数(SLEDAI)结果分为活动期组和稳定期组,比较两组患儿TB细胞亚群的差异,并分析与SLEDAI、补体的相关性。结果SLE患儿白细胞、外周血淋巴细胞绝对值及百分比、NK细胞绝对值及百分比、T淋巴细胞绝对值、CD4^(+)T细胞绝对值、记忆CD4^(+)CD45RA-T细胞绝对值及百分比、纯真CD4^(+)T细胞绝对值及百分比、CD4/CD8比值较健康儿童显著降低。B淋巴细胞百分比、T淋巴细胞百分比、CD8^(+)T细胞百分比、45RA+/CD4^(+)T细胞绝对值及百分比明显增高,差异均有统计学意义(P<0.01)。处于疾病活动状态的SLE患儿淋巴细胞百分比显著降低[(27.2%±12.1%)vs(32.5%±16.1%),P<0.05]。相关性分析显示,SLEDAI评分与淋巴细胞百分比呈负相关(r=-0.23,P<0.01),补体C3与淋巴细胞百分比(r=0.27,P<0.01),淋巴细胞绝对值(r=0.23,P<0.01),自然杀伤细胞绝对值(r=0.27,P<0.01),T淋巴细胞绝对值(r=0.23,P<0.01),CD4^(+)T细胞绝对值(r=0.29,P<0.01),45RA+/CD4^(+)T细胞绝对值(r=0.22,P<0.05),纯真CD4^(+)T细胞绝对值(r=0.23,P<0.05)均呈正相关,补体C4与淋巴细胞百分比呈正相关(r=0.27,P<0.01)。结论TB细胞亚群失衡参与了儿童SLE的发生,其可作为SLE患儿疾病活动的一项参考指标,检测SLE患儿的外周血TB细胞亚群的变化,对判断病情具有重要意义。 Objective To analyze differences in peripheral blood TB cell subsets between children with systemic lupus erythematosus(SLE)and healthy children,and to explore correlation between TB cell subsets and lupus disease activity.Methods Flow cytometry was employed to detect the lymphocyte subsets of children with SLE(n=129)and healthy children(n=195).1∶1 matching was performed according to gender and age.A total of 69 pairs of children were successfully matched,and differences in TB cell subsets between the two groups of children were compared.SLE children were divided into active phase group and stable phase group based on SLE disease activity index(SLEDAI)results.Differences in TB cell subsets between the two groups were compared,and correlation with SLEDAI and complement was analyzed.Results In children with SLE,white blood cell count,the absolute value and percentage of peripheral blood lymphocytes,the absolute value and percentage of NK cells,the absolute value of T lymphocytes,the absolute value of CD4^(+)T cells,the absolute value and percentage of memory CD4^(+)CD45RA-T cells,absolute value and percentage of naive CD4^(+)T cells,and CD4/CD8 ratio were all significantly lower than those in healthy children.The percentages of B lymphocytes,T lymphocytes,and CD8^(+)T cells,as well as the absolute count and percentage of 45RA+/CD4^(+)T cells significantly increased,and difference was statistically significant(P<0.01).In children with SLE who were in active disease states,the percentage of lymphocytes significantly decreased([27.2%±12.1%]vs.[32.5%±16.1%],P<0.05).Correlation analysis showed that the SLEDAI score was negatively correlated with the percentage of lymphocytes(r=-0.23,P<0.01),while complement C3 was all positively correlated with the percentage of lymphocytes(r=0.27,P<0.01),the absolute count of lymphocytes(r=0.23,P<0.01),the absolute count of natural killer(NK)cells(r=0.27,P<0.01),the absolute count of T lymphocytes(r=0.23,P<0.01),the absolute count of CD4^(+)T cells(r=0.29,P<0.01),the absolute count of 45RA+/CD4^(+)T cells(r=0.22,P<0.05),and the absolute count of naive CD4^(+)T cells(r=0.23,P<0.05).Complement C4 was positively correlated with the percentage of lymphocytes(r=0.27,P<0.01).Conclusion The imbalance of TB cell subsets is involved in the development of SLE among children,which can be served as a reference indicator for disease activity in children with SLE.Monitoring changes in peripheral TB cell subsets in children with SLE is of great significance for assessing disease condition.
