基于“三焦气化”理论探讨支气管哮喘的中医药防治原则

摘要 基于三焦气化理论探讨支气管哮喘的中医药防治原则,指出三焦气化失权是支气管哮喘多脏腑气阳亏虚的根本病机,也是气机逆乱,痰瘀伏肺的根源。哮喘发作期以振奋阳气,涤痰祛瘀,通调三焦为基础;缓解期以荡涤余邪,补虚固本,通调三焦为关键。温阳的益气要诀为通调三焦,即通调上焦宣降肺气,通调中焦温运脾胃,通调下焦摄纳肾气。三焦气化通畅则气阳得以化生,痰、瘀等病理产物得以消除。 Therapeutic principles of bronchial asthma(BA)in TCM are discussed based on the theory of Qi transformation of three energizers,it is believed that failure of Qi transformation of three energizers is the basic pathogenesis for bronchial asthma with Qi-Yang deficiency of many viscera,and the root origin of Qi movement inversion,and phlegm and blood stasis in the lung.At the stage of attack,revitalizing Yang,resolving phlegm and removing stasis,dredging and regulating three energizers is adopted as the fundamental therapy;at the remission stage,washing away the remaining evils,invigorating the weak and strengthening the root cause,as well as dredging and regulating three energizers is used as the crucial therapy.The key point of benefiting Qi for warming Yang is to dredge and regulate three energizers,that is,dredging and regulating the upper energizer means dispersing and descending lung Qi,the regulation of the middle energizer is to warm and transport spleen and stomach,adjusting the lower energizer is to control kidney Qi.Smooth Qi transformation of three energizers could maintain the transformation and production of Qi-Yang,and eliminate pathological products such as phlegm and stasis.
