摘要
目的:探讨指背血管蒂岛状皮瓣转移修复手部软组织缺损的围术期护理体会及护理效果。方法:选取2019年3月-2023年3月笔者医院收治的80例行指背血管蒂岛状皮瓣转移修复手部软组织缺损的患者,按照随机数字表法分为观察组(n=40)和对照组(n=40)。对照组给予常规护理,观察组给予针对性护理。统计比较两组患者护理后的手功能恢复情况[密歇根手功能评分(Michigan hand outcomes questionnaire,MHQ)]、创面修复外观、瘢痕评分[温哥华瘢痕量表(Vancouver scar scale,VSS)]、心理状态[焦虑自评量表(Self-rating anxiety scale,SAS)和抑郁自评量表(Self-rating depression scale,SDS)]、并发症发生情况及护理满意度[纽卡斯尔护理满意量表(Newcastle nursing satisfaction scale,NSNS)]。结果:护理后,观察组MHQ各维度评分均高于对照组,创面修复外观优于对照组,并发症发生情况、瘢痕评分、SAS、SDS评分均低于对照组,差异均有统计学意义(P<0.05)。观察组护理满意度高于对照组(P<0.05)。结论:围术期采用针对性护理模式能更好地恢复指背血管蒂岛状皮瓣转移修复手部软组织缺损后的手功能,改善创面外观和患者心理状态,减少术后并发症的发生,提升护理满意度。
Objective To explore perioperative nursing experience and nursing effect of dorsal digit island flaps transfer in the repair of hand skin defects.Methods A total of 80 patients undergoing dorsal digit island flaps transfer for the repair of hand skin defects in the author's hospital were enrolled between March 2019 and March 2023,and they were divided into observation group (n=40) and control group (n=40) according to random number table method.The control group was given routine nursing,and the observation group was given targeted nursing.The recovery of hand function[Michigan hand outcomes questionnaire(MHQ)],wound repair appearance,scar score[Vancouver scar scale (VSS)],psychological state[self-rating anxiety scale (SAS) and self-rating depression scale (SDS)],occurrence of complications and nursing satisfaction[Newcastle nursing satisfaction scale (NSNS)]were compared between the two groups after nursing.Results After nursing,the MHQ score of the observation group was higher than that of the control group in all dimensions,the appearance of wound repair was better than that of the control group,and the incidence of complications,scar score,SAS and SDS scores were lower than those of the control group,with statistical significance (P<0.05).The nursing satisfaction of observation group was higher than that of control group (P<0.05).Conclusion Perioperative targeted nursing can better recover hand function in patients after dorsal digit island flaps transfer for the repair of hand skin defects,improve wound appearance and psychological state,reduce the occurrence of postoperative complications and increase nursing satisfaction.
