摘要
目的探讨经阴道超声(TVS)检查、多模态磁共振成像(MRI)、氟化脱氧葡萄糖-正电子发射计算机断层显像/计算机断层扫描(^(18)F-FDG PET/CT)在子宫内膜癌(EC)诊断中的应用。方法选取EC患者157例,记录多模态MRI及TVS的EC分期检查结果以及^(18)F-FDG PET/CT阳性检查结果,并与病理结果进行比较。结果TVS检测结果显示,Ⅰ期87例,误诊6例,漏诊12例;Ⅱ期55例,误诊12例,漏诊8例;Ⅲ期15例,误诊2例,无漏诊。多模态MRI检测结果显示,Ⅰ期86例,误诊3例,漏诊10例;Ⅱ期58例,误诊10例,漏诊3例;Ⅲ期13例,无漏诊误诊。^(18)F-FDG PET/CT检测结果显示,阳性患者145例,漏诊12例。以术后病理分期结果为金标准,TVS诊断EC患者Ⅰ期准确率为87.10%(81/93),Ⅱ期准确率为84.31%(43/51),Ⅲ期准确率为100.00%(13/13)。以术后病理检查分期结果为金标准,多模态MRI诊断EC患者Ⅰ期的准确率为89.25%(83/93),Ⅱ期准确率为94.12%(48/51),Ⅲ期准确率为100%(13/13)。以术后病理分期结果为金标准,^(18)F-FDG PET/CT检测的EC阳性患者145例,准确率为92.36%(145/157)。结论^(18)F-FDG PET/CT对EC患者的诊断准确率较高,TVS、多模态MRI对EC术前分期诊断有较高价值,值得临床推广使用。
Objective To investigate the application of transvaginal ultrasound(TVS),multimodal magnetic resonance imaging(MRI)and ^(18)F-fluorodeoxyglucose positron emission computed tomography/computed tomography(^(18)F-FDG PET/CT)in diagnosis of endometrial carcinoma(EC).Methods A total of 157 patients with endometrial cancer in our hospital were selected.The results of EC staging by multimodal MRI and transvaginal ultrasound and positive ^(18)F-FDG PET/CT were recorded,and their results were compared with pathological results.Results The results of TVS showed 87 cases in stageⅠ,6 cases were misdiagnosed and 12 cases were missed diagnosis.In 55 cases of stageⅡ,12 cases were misdiagnosed and 8 cases were missed.In 15 cases of stageⅢ,2 cases were misdiagnosed without missed diagnosis.The results of multimodal MRI showed that 86 cases were misdiagnosed in stageⅠ,3 cases were misdiagnosed and 10 cases were missed.In 58 cases of stageⅡ,10 cases were misdiagnosed and 3 cases missed diagnosis.There were 13 cases of stageⅢwith no missed diagnosis or misdiagnosis.The results of ^(18)F-FDG PET/CT were positive in 145 cases and missed in 12 cases.Based on the golden standard of postoperative pathological staging,the accuracy of TVS diagnosis of EC patients in stage I was 87.10%(81/93),stageⅡwas 84.31%(43/51),and stageⅢwas 100%(13/13).Based on the golden standard of postoperative pathological staging,the accuracy of multimodal MRI in the diagnosis of EC patients was 89.25%(83/93)in stageⅠ,94.12%(48/51)in stageⅡ,and 100%(13/13)in stageⅢ.According to the pathological staging,145 EC positive cases were detected by ^(18)F-FDG PET/CT,the accuracy rate was 92.36%(145/157).Conclusion ^(18)F-FDG PET/CT has a high diagnostic accuracy in EC patients,TVS and multimodal MRI have a high value in the preoperative staging diagnosis of EC,and are worthy of clinical promotion.
