普萘洛尔联合小剂量^(131)I治疗Graves甲亢的临床效果

摘要 目的探讨普萘洛尔联合小剂量^(131)I治疗Graves甲亢的临床效果。方法选择2021年1月至2023年1月本院收治的80例Graves甲亢患者作为研究对象,以随机数字表法将其分为对照组和研究组,每组40例。对照组接受小剂量^(131)I治疗,研究组给予普萘洛尔联合小剂量^(131)I治疗。比较两组的甲状腺功能指标、骨代谢指标、肝功能指标、临床疗效以及不良反应发生情况。结果治疗后,研究组的促甲状腺激素(TSH)水平明显高于对照组,游离三碘甲状腺原氨酸(FT_(3))、游离甲状腺素(FT_(4))水平明显低于对照组(P<0.05)。治疗后,研究组的血钙、骨钙素水平明显低于对照组(P<0.05)。治疗后,研究组的谷草转氨酶(AST)、丙氨酸氨基转移酶(ALT)及总胆红素(TBIL)水平显著低于对照组(P<0.05)。研究组的治疗总有效率明显高于对照组(P<0.05)。研究组的不良反应总发生率略高于对照组,但差异无统计学意义(P>0.05)。结论普萘洛尔联合小剂量^(131)I治疗Graves甲亢能有效改善患者的甲状腺和骨代谢功能,减轻肝损伤,且疗效明显、安全性较高,值得在临床中应用及推广。 Objective To investigate the clinical effect of propranolol combined with low-dose^(131)I in the treatment of Graves hyperthyroidism.Methods A total of 80 patients with Graves hyperthyroidism admitted in our hospital from January 2021 to January 2023 were selected as the research objects.The patients were divided into control group and study group by random number table method,with 40 cases in each group.The control group was treated with low-dose^(131)I,and the study group was treated with propranolol combined with low-dose^(131)I.The thyroid function indexes,bone metabolism indexes,liver function indexes,clinical efficacy and adverse reactions were compared between the two groups.Results After treatment,the level of thyroid stimulating hormone(TSH)in the study group was significantly higher than that in the control group,and the levels of free triiodothyronine(FT_(3))and free thyroxine(FT_(4))were significantly lower than those in the control group(P<0.05).After treatment,the levels of serum calcium and osteocalcin in the study group were significantly lower than those in the control group(P<0.05).After treatment,the levels of glutamic oxaloacetic transaminase(AST),alanine transarninase(ALT)and total bilirubin(TBIL)in the study group were significantly lower than those in the control group(P<0.05).The total effective rate of treatment in the study group was significantly higher than that in the control group(P<0.05).The total incidence of adverse reactions in the study group was slightly higher than that in the control group,but the difference was not statistically significant(P>0.05).Conclusion Propranolol combined with low-dose^(131)I in the treatment of Graves hyperthyroidism can effectively improve the thyroid and bone metabolic function of patients,reduce liver injury,and has obvious curative effect and high safety,which is worthy of clinical application and promotion.
