摘要
目的 分析咪达唑仑注射液从第二类精神药品调整为第一类精神药品对天津医科大学总医院静脉镇静药物使用情况的影响,为临床安全、有效、经济使用静脉镇静药物提供参考。方法 提取天津医科大学总医院2023年7至12月、2024年1至6月(调整前)和2024年7至12月(调整后)住院患者静脉镇静药物的使用数据,对各类静脉镇静药物的消耗量、用药频度(DDDs)、咪达唑仑注射液科室使用变化等进行回顾性分析。结果 执行管理新规后,咪达唑仑注射液的消耗量同比下降18.38%,环比下降34.10%,其余静脉用镇静药物环比均有不同程度上涨,尤其是注射用甲苯磺酸瑞马唑仑。管理调整对于静脉用镇静药物DDDs排名没有影响,但是执行管理新规后,咪达唑仑注射液DDDs及占比呈下降趋势。管理调整前后咪达唑仑注射液DDDs排名前三名科室没有变化。结论 药物管理政策的调整会对临床用药产生一定影响,有助于进一步规范临床合理用药。
Objective To analyze the impact of adjusting midazolam injection from the second-class psychotropic drug to the first-class psychotropic drug on the use of intravenous sedative drugs in Tianjin Medical University General Hospital,and to provide a reference for the safe,effective,and economical use of intravenous sedative drugs in clinical practice.Methods The usage data of intra-venous sedative drugs for inpatients in Tianjin Medical University General Hospital from July to December 2023,January to June 2024(before the adjustment),and July to December 2024(after the adjustment)were extracted.A retrospective analysis was conducted on the consumption volume of various intravenous sedative drugs,defined daily doses(DDDs)and the changes in the using departments.Results Affer the implementation of the new management regulations,the consumption of midazolam injection decreased by 18.38%year-on-year and by 34.10%month-on-month.The consumption of other intravenous sedative drugs increased to varying degees month-on-month,especially the injection of remimazolam tartrate.The management adjustment had no impact on the ranking of DDDs of intravenous sedative drugs.However,after the implementation of the new management regulations,the DDDs and proportion of mid-azolam injection showed a downward trend.Before and after the management adjustment,the top three departments for midazolam in-jection DDDs remained unchanged.Conclusion The adjustment of drug management policies will have certain impacts on clinical medi-cation use,and it helps to further standardize clinical rational drug use.
