摘要
目的:探究钬激光碎石技术在输尿管上段结石患者中的应用价值。方法:选取100例输尿管结石患者(于2021年1月至2023年12月收治),以手术方法的不同将其分为A、B两组,每组各50例。A组、B组分别应用输尿管硬镜钬激光碎石术、输尿管软镜钬激光碎石术治疗,比较两种术式的治疗效果及安全性。结果:两组手术时间、术中出血量相比,差异无统计学意义(P>0.05)。B组术后血尿时间较A组短(P<0.05)。A组总碎石成功率为92.00%(46/50),B组总碎石成功率为98.00%(49/50),两组相比,差异无统计学意义(P>0.05)。两组即刻结石清除率、总结石清除率相比,差异无统计学意义(P>0.05)。B组并发症发生率较A组低(P<0.05)。手术后回院复查时,两组健康状况调查简表(SF-36)评分均较手术前高,且B组高于A组(P<0.05)。结论:应用输尿管软镜、硬镜钬激光碎石术治疗输尿管上段结石的效果相近,但前者可缩短术后血尿时间,降低并发症的发生率,提高患者的生活质量。
Objective:To explore the application value of holmium laser lithotripsy technology in patients with upper ureteral stones.Methods:A total of 100 patients with ureteral stones admitted from January 2021 to December 2023 were selected and divided into group A and group B according to different surgical methods,with 50 cases in each group.Group A was treated with holmium laser lithotripsy by rigid ureteroscopy,while group B was treated with holmium laser lithotripsy by flexible ureteroscopy.The therapeutic effects and safety of the two surgical procedures were compared.Results:There was no statistically significant difference(P>0.05)in surgical time,intraoperative blood loss,total stone fragmentation success rate,and total stone clearance rate between the two groups.Compared with group A,group B had a shorter postoperative hematuria time and a lower incidence of complications(P<0.05).On the first day after surgery,the levels of malondialdehyde(MDA)and cortisol(Cor)in both groups were higher than before surgery,while the levels of MDA and Cor in group B were lower than those in group A(P<0.05).During postoperative followup,the SF-36 scores of both groups were higher than before surgery,and group B was higher than group A(P<0.05).Conclusion:The above two treatment methods have similar therapeutic effects,but the method of group B can shorten the postoperative hematuria time,reduce the incidence of complications and the early physiological stress reaction after surgery,and improve the quality of life of patients.
